Summary:

The replacement of teeth lost by children because of trauma can be an important indication for early implant therapy. Osseointegrated dental implants, like ankylosed teeth, alter position as growth-related changes occur within the jawbones (displacement, remodeling, mesial drift). Facial growth of the child and even of the adolescent, as well as the continuous eruption of the adjacent anterior teeth, create significant risk of a less favorable esthetic and/or functional outcome. For patients with a normal facial profile, the placement of an implant should be postponed until growth is complete. For patients with a short or long face type, further growth, especially the continuous eruption of adjacent teeth, creates a serious risk even after the age of 20 years, as illustrated by some recent clinical studies. This review aims to explain these phenomena and provides some recommendations for implant placement. INT J ORAL MAXILLOFAC IMPLANTS 2006;21:867–878

Immediate implant therapy involving implants placed into intact Type 1 extraction sockets has become a consistent clinical technique. The classification of Type 2 extraction sockets, where the mucosal tissues are present but there is a midfacial osseous dehiscence defect, has been described according to the extent of the buccal bone plate absence. The literature has offered different techniques in the treatment of Type 2 sockets; however, the extent of the defect has never been defined or delineated.

Bone Augmentation vs Short Implants: Making Clinical Decisions for Tooth Replacement at Sites with Alveolar Ridge Atrophy
There is often some degree of atrophy in sites planned for implant placement. An array of therapeutic options is available for implant placement in sites with vertical atrophy of the alveolar ridge, including ridge augmentation, sinus augmentation in the maxilla, nerve repositioning in the mandible and tilted implants. Short implants have been systematically evaluated as an alternative in sites with vertical alveolar ridge atrophy with favorable clinical outcomes comparable to standard-length implants. Decision making for selection of appropriate therapeutic modality for sites with vertical alveolar ridge atrophy has to consider many theoretical, surgical and prosthetic considerations. Surgical and prosthetic guidelines will be provided for application of short implants for posterior sites with limited vertical height.
