Summary:

Dentistry has entered an era in which patients no longer need to accept an edentulous or partially edentulous condition, or one in which their candidacy for tooth structure replacement (ie, implants with subsequent restoration) must be dismissed because of insufficient alveolar bone volume, height, or width. The supporting bone can be preserved at the time of tooth extraction, or augmented at the time of case presentation, using a variety of available regenerative materials. Among them are mineralized human allograft bone and collagen membranes that can be placed in combination with specific growth factor complexes and implant designs. This article reviews the challenges associated with adequately preserving or augmenting the alveolar bone after tooth extraction or loss and before implant placement. The research and benefits to support using allogenic bone graft and membrane materials for such procedures are detailed, and 3 clinical cases are presented to demonstrate the clinically successful incorporation of these materials with the host tissues.

Atrophic Posterior Maxillan and Mandible: Alveolar Ridge Reconstruction with Mandibular Block Autografts
Autogenous bone grafts have been used for many years for alveolar ridge augmentation and are still considered to be the gold standard for jaw reconstruction. The use of these grafts with osseointegrated implants was originally discussed by Brånemark et al., who used the iliac crest as the donor site. For repair of most localized alveolar defects, however, bone grafts from the mandible offer advantages over iliac crest grafts. These include the proximity of donor and recipient sites, convenient surgical access, decreased donor site morbidity and decreased cost. This article focuses on posterior maxillary and mandibular osseous augmentation in a staged approach for implant placement. Both horizontal and vertical deficiencies are addressed with the use of symphysis and ramus buccal shelf donor block bone via case presentations.

This lecture is focused on an effective regenerative approach, but at low knowledge. The goal is to provide a new skill in favor of well-established techniques. The Fibrin Sealant (FS) is an additional clot of human origin. It has adhesive, sealant and inductive action stimulating hard and soft-tissue regeneration. The FS, mixed with bone chips and biomaterials, allows a better handling of the graft that, becoming plastic and moldable, adapts perfectly to bony defects without dispersion of granules. It’s effective also on wound protection achieving a faster healing of soft-tissue. This approach is excellent in the socket preservation and in the peri-implant regeneration on "closed" sites where the implants are inside the defects.
