dear colleagues, as we see there is some open bite and space between anteriors and non harmonic gingival line or pink smile line which can be corrected conservatively only by orthodontics and discolorations which seems fluorosis can be decreased by micro-abrasion and the definite treatment will be PLV . in my opinion other treatments without ortho treatments will be compromised.

If you want to treat this case in a minimally invasive manner. I would recommend: - Microabrasion (Opalustre) - Bleaching - home or in surgery - GC MI Paste plus - Minimal or no preparation veneers if patient requires further aesthetic improvement - Optional depending on patient demands - short term ortho to close anterior open bite

Direct bonding. Be aware with the etching times, especially in healthy enamel so yo can have good adhesion. Best regards and results! Post the final photos!!

as the angulation of the teeth is favourable you can proceed with the case with porcelain veneers to have the best esthetic results but u have to cosider the economic situation and u have to consider the gingival level as i see the patient needs esthetic crown lengthening

Contact you GC America rep regarding the protocol for remineralizing white spots using MI Paste. The process involves etching the teeth and applying MI Paste in a custom tray. Some of the results they show with this protocol have been pretty impressive.

Porcelain veneers are the very best option because they will transform the patient's smile whereas bleaching or microabrasion will not AND there will not be a need for tooth preparation due to angulation of the posteriors which need to be built out unless the patient will undergo orthodontics to improve the bite.

If the patient doesn't want any tooth preparation, I'd manage the discolorations with Opalustre from ultradent, a chemical and mechanical microabrasion system. Read the instructions provided by the manufacturer. After one or maybe two microabrasion treatments, I'd continue with take home whitening treatment for two weeks. Remember that hypomineralization areas (bright white spots) may appear brighter after bleaching so it may need some esthetic bonding taking into account that if not any preparation is perform, the restoration will look overcontoured and thus compromising the final result. I strongly recommend an opaque composite as a deep dentin effect to opaque the discolorations.
