On the CBCT image, I think we need block graft for vertical and lateral bone augmentation because we have a big defect there. But there's no bone (root exposed) at neighbor teeth, how can the graft work?

Very challenging case and a number of options have been discussed. I would work on the soft tissue first, do a connective tissue or a combined graft with the epithelised portion on the ridge and the connective tissue to cover the root and bulk up laterally. The problem I anticipate at this juncture without addressing the soft tissue first is that you'll have tension on your closure. 6-8 weeks after the soft tissue procedure, you'll be able to perform either a block for vertical augmentation or the GBR with a mesh much more comfortably.All the best with the case and keep us posted on the progress. | | Register For Free To Reply |

| 6/30/2012 11:38:34 PM | Maurice Salama | | Require to do Hard and soft tissue augmentation. | | Would love to evaluate the CBCT but my guess would be GBR with Ti-Mesh or Curve, tenting screws to maintain space as well as bio active modifier or BMP-2 in combination with mineralized allograft.
Follow this with CTG and further grafting as needed at time of implant placement.
