According to the instruction of the product infuse. It is not recommended to extra grafting material onto the carrier. However, I saw a lot of dentists added FDBA onto the collagen carrier. What's your opinion?

Yes, Harry. It is not recommended to add bone to the Collagen sponges with BMP-2.
However, Ridge Augmentation is an off-label use of the product and many clinicians
are suggesting to add bone to the sponges to aid in the regeneration.
I would suggest mineralized irradiated cortical-cancelous allograft as an additional filler and space maintainer.
I would avoid demineralized materials or xenografts as they may interfere with healing with the BMP-2.
It is early yet in this regard and we are hopeful that more information will be available soon to help establish a more definitive protocol for use in ridge augmentation.

Dear Maurice, Partners in Synergy The Ultimate Grafting Course Was AMAZING! I am very excited to implement many of the techniques that you and Dr. Pikos taught in Orlando, Thank You Both ,for such a GREAT EXPERIANCE.

In this type of case if I did not have acess to a titanium mesh could I use Dynablast bone putty which is I believe a combination of Demineralized cortical ( suposedly Osteogenic) and mineralized cancelous bone? Over that I would use a Biomend extend or a goretex like membrane with tacs. Also a technical question about the Buccal flap is does it become split thickness past the attached tissue or mucogingival junction? Is that how you would create enough tension free closure to allow primary and complete closure of your tissues over the membrane?

Just a Proposition: Can we
A. TRIM the dimensions of the titanium mesh at the borders to the "SPECIFICALLY NEEDED/targeted AREAS" for bone growth(to support and maintain implants),
B. Create "ACCESS HOLES" in the mesh for implants,
C. Achieve "FIXATION of the MESH" utilising tacks, say, at distal ridge crest, buccal cortical plate, or even in between the implants at a suitable location on the buccal,
D. Remove the fixation screws after the planned period of time lapse, and
E. LEAVE the mesh in place for PERMANENT.
F. What would be consequence/s if any, upon the mesh "left in situ" indefinitely?

A. You can trim the mesh as you wish.
B. Yes, create access holes much like the FT-Wing from Aki Funato.
C. Yes, fixate in at least 2 spots.
D. Yes, remove fixation screws or not if away from sulcus envt.
E. Have not tried it but can't see why not? Could be color problem for gingiva in esthetic.
F. The mesh can "move" and exfoliate or become infected if left in place.
Exciting thoughts for the future.

I always enjoy your presentations great videography and idealized clinical examples which enable trans mission of the basics for decision making.I would like to order the Infuse( BMP sponge) I have heard from the orthopedic surgeons how well the product grows bone in the spine.Please forward the information so I may order the material. You should have a rep from the company come and see you to describe the dosage and utilization in clinical practice.

Hello and thanks for your post. I am re-entering this case in the next 2 months and will update it on this site for all to see.
The technique is not so unusual but the utilization of BMP-2 is somewhat new for ridge augmentation. I am hopeful that we will see good results as this would be easier for both the patients and the treating surgeon.
