Modern Bone Augmentation Techniques: An Alternative Donor Site
In this lecture, Dr. Glickman will give the participants an alternative bone block harvesting site that is in the same area as the anterior maxillary teeth, eliminating the need for a second site surgery. This also provides a site that is painless and easily accesible therefore allowing the patients to have the gold standard of bone grafting rather than settling for treatment options with a more more variable outcome.

Crestal Approach Sinus Augmentation with Osteotome Technique and Cartridge Delivery System
There are several ways to elevate the sinus membrane when accessed through the crest of the ridge and deliver graft into the sinus. Summers’ osteotome technique is one of the most widely used surgical procedures to achieve sinus elevation. One of the biggest challenges is instrumentation and grafting through the prepared osteotomy site. In this video, Dr. Kher will demonstrate the new cartridge delivery system from NovaBone that considerably reduces site preparation and facilitates elevation of the sinus membrane with the hydraulic pressure of Putty as it is delivered into the sinus.

Total Rehabilitation of the Edentulous Patient with the All-on-4® treatment concept
A fixed implant supported prosthesis is the ideal treatment option for the edentulous patient and those facing the imminent loss of their remaining dentition. Through innovation, along with continuous clinical and scientific research, it has become possible to treat virtually every edentulous case with a highly functional and esthetic fixed solution. The oral rehabilitation of these patients has two stages: surgical and prosthetic. The surgical protocol follows the All-on-4® treatment concept - an innovative technique developed in the early 90’s at the MALO CLINIC that allows for immediate function, full-arch, implant supported prosthetics.

Managing the Extraction Site - Optimal Preservation and Enhancement Techniques
There is rarely a day that clinicians do not have to deal with an extraction site. If implant therapy is a consideration, then bone and soft tissue quantity and quality must be factored into treatment options. The ideal opportunity for bone preservation exists at the time of tooth extraction. All too often this opportunity is lost. Extraction techniques will be demonstrated that not only preserve but augment the amount of alveolar bone. The importance of piezosurgery, atraumatic surgical technique, the use of bone grafts, resorbable membranes and growth factors will be elucidated along with soft tissue enhancement.

Zygomatic Implants in the Treatment of Fully Edentulous Patients
This presentation aims to bring some possibilities in the treatment of edentulous patients with a severe atrophic maxilla. The major problem is that the absence of an appropriate bone bed, will compromise any approach with bone grafts and substitutes, as well as involving a greater morbidity and treatment time. In this way the oral rehabilitation with zygomatic implants is a current need for many patients, which will require a scientific knowledge of the dental care professional. Through this slide show and video demonstration you will be able to recognize and diagnose cases recommended for the therapy with zygomatic implants and its possible configurations supporting an oral rehabilitation.

The Hidden Value of Digital Radiography for Caries Detection
Digital radiography allows the dentist to use powerful image analysis tools to extract information from the image that might otherwise be missed by visual inspection alone. Historically interproximal caries detection is done by visually inspecting the radiograph for evidence of a radiolucency produced by the decay and visually determining if the decay is deep enough that a restoration is required.

Improving the Dentist/Technician Communication for optimum Smile Design and Ceramic Restorations
As modern dentists and dental technicians we need to keep abreast of the latest developments in dentistry and incorporate new techniques and materials into our armamentarium on an ongoing basis. Moreover, it is essential that all the team members participate proactively in the treatment planning process in order to ensure the best possible result for the benefit of the patient. Consequently, consistent interaction among the members of the dental team is required. Technicians should have a basic understanding of clinical procedures and the development of target-oriented communication protocols are essential prerequisites for a smooth workflow and the provision of high-quality service. Dentists should be aware that crucial treatment steps such as the diagnostic wax-up, the fabrication of the mock-up, shade selection, material selection, prep design, and the fabrication of temporary restorations, etc. can be greatly improved by taking into account the technician’s perspective.

The Merger of Orthodontics with Cosmetic Dentistry
As orthodontists, we don’t do laminates. But when we are finishing a case, sometimes the smile just doesn't “look right”. But not all our patients either want or can afford laminates. Not all even need them to get a great smile. So, what principles can we learn from the cosmetic dentists and use in our orthodontic cases? Apparently quite a lot! In this presentation, Dr. Sarver will illustrate finishing procedures such as reshaping contacts, connectors, embrasures, and even soft tissue contouring with lasers to achieve that next level of smile presentation in your orthodontic cases.

Tissue Engineering and Platelet Derived Growth Factors: Evidence Based Therapy
Platelet derived growth factors are now routinely utilized in reconstructive therapy. This presentation describes very detailed and evidence based guidelines for clinicians interested in enhancing their abilities in tissue engineering, especially as it relates to bone augmentation. Specifically, while successful bone augmentation requires the standard surgical parameters of space maintenance, low pressure on the grafts and tension-free flap closure, optimization of this goal requires management and enhancement of the local biological conditions with growth factors. Towards that end, platelet concentrates through the constant release of growth factors are able to promote and enhance new vascularization, provide plasma protein, normal lipidemia, as well as increased collagen and fibrin activity.

TEETH 2013: When to Save and When to Extract and When to Regenerate
The treatment of periodontally compromised patients has been significantly altered by the availability of predictable implant dentistry.Where in the past, a compromised tooth with advanced bone loss and/or furcation invasion might have been retained, the high success rate of osseointegrated implants challenges the clinical decision. Should the tooth be saved with periodontal regeneration or be replaced with an implant?
Conversely, new biomimetic mediators have enhanced the predictability of regenerative outcomes and encourage endeavors to maintain the natural dentition. This lecture will address the clinical decision that occurs in every practice: save the tooth or place an implant.

A Clinical Update in Bone Regeneration: Autogenous Bone vs. BMP-2
For historical and biological reasons autogenous bone has long been considered the gold standard of bone graft materials. For bone augmentation procedures autogenous grafts provided predictable volume gains, favorable bone quality with short healing periods. However, the need to harvest the graft and associated morbidity are inherent disadvantages. The choice of a technique for bone augmentation is dependent on a number of factors including the use of bone substitutes. More recently recombinant human bone morphogenetic protein (rhBMP-2) has been shown to be effective in repairing buccal wall defects from extractions and in sinus bone grafting. The use of rhBMP-2 is currently under clinical investigation as a replacement for autograft in ridge augmentation process.

Prosthetic Strategies to Enhance Dental Implant Aesthetics
The principle focus of implant dentistry has changed significantly from the era of ‘Anatomically driven implant placement’ to the current practice of ‘Restoration driven implant placement’. When we now look at the failing dentition in the aesthetic zone we now must immediately consider conversion to implants. We are continually seeking greater control of the peri-implant tissues and ultimately the aesthetics of the restoration. Implant/abutment designs allow us to consider immediate implant replacement, and immediate non-functional loading especially in the aesthetic zone, where preservation of hard and soft tissue is of paramount importance in order follow the principles of aesthetics by maintaining the elements which are already there

Esthetic Techniques and Materials
Since the mid 1990’s more and more people have sought out the Dentist for elective cosmetic solutions. The esthetic revolution is now in full swing, and as diseased based dentistry continues to decline, successful dental teams have embraced elective dental care, and are seeing it make dramatic differences in patient’s lives. Additionally, the baby boomer population is driving the current marketplace. Unlike there parents they expect to keep there teeth for a lifetime, and yet will be living longer then any other generation in history. Having a thorough understanding of esthetics, occlusion, the use of the latest restorative materials, as well as the behavioral understanding of this population is critical for success.

Pre-Implant Ridge Bioengineering: The Bone Block Allograft Technique - Part 2 of 2
Narrow ridge deficiencies can limit dental implant placement. As well, the pain and various complications associated with chin, ramus and hip bone block grafts for ridge augmentation purposes discourage many from pursuing this method of treatment. The 'bone block allograft technique' helps overcome narrow ridge limitations for dental implant placement as well as minimizing surgical post-op pain, risks and complications. This is an extremely versatile procedure which can be performed in the anterior and posterior sextants of both the maxillary and mandibular arches. The Free Gingival Graft...Back to Basics: Why Do It?
In this video, a free gingival graft is demonstrated to address a recession defect as well as to augment the soft tissue while eliminating the high frenum pull in order to aid in the orthodontic treatment plan of a young patient. While commonly shunned for the morbidity associated with palatal grafts, the free gingival graft is making a comeback with the popularity of implant dentistry due to it's relative simplicity and predictability for augmenting thin tissue. This "back to basics" video takes the free gingival graft back to its roots: The lower incisor area.

Titanium Mesh and it's Role in Bone Augmentation; A Clinical Review
Different surgical procedures have been proposed in reconstruction of deficient alveolar ridges for placing dental implants in optimum prosthetic and esthetic position. One of the inconveniences of titanium mesh which has been reported is the exposure of it but on the other hand the benefit in comparison to non resorbable membranes is that there is no need for removal and the risk of infection is very low. In this lecture different cases will be presented and clinical points to be considered in using titanium mesh will be highlighted.

Gaining Horizontal and Vertical Bone with Mineralized Bone Particulate, rh-BMP2 and Resorbable Ultrasonic-Applied Rigid Barriers
The use of titanium mesh has been well described, but so has the complications of removal and exposure. Bioresorbable barriers are now available which can be molded into any desired three-dimensional form, creating a rigid and resorbable construct that is secured to the bone using ultrasonic energy. This unique solution has been successfully used to create the proper environment for both large lateral and vertical ridge augmentations for situations previously only treated with block grafting or distraction. Dr. Cummings will share his experience as well as the key soft tissue techniques that are critical for the success of any large augmentation.

Immediate Molar Replacement Utilizing Narrow Diameter Implants - Allowing Implant Placement in Almost Any Situation - Part 2 of 2
Immediate molar placement has been widely reported in the literature with excellent results. All the studies insist that a wide diameter implants are essential as they are the only way to achieve primary stability and are the only implants that can handle the forces that molar regions produce. But there are a number of problems with wide diameter implants. This presentation will dispel all these myths as well as show how narrow diameter implants can not only handle the forces but also make immediate molar placement more predictable and possible in almost all scenarios.
