Microscopic Dentistry; Practical Use in Restorative Dentistry Today
Restorative dentistry has changed in recent years. New materials and techniques allow the execution of partial restorations, direct and indirect, with metal-free techniques and started a true partnership between this discipline and prosthodontics. A good esthetic and functional rehabilitation starts from the proper clinical evaluation of the patient and of his problems, planning all the clinical and laboratory procedures in order to reduce the possibility of performing a subpar restoration. The use of high magnification in the right steps can improve our clinical result and optimize the session with the patient. The goal is to achieve the right balance between biology, esthetic and function by applying a minimally invasive approach.

Vertical or Shoulderless Preparations in Contemporary Prosthodontics Premium Member Content

Vertical or Shoulderless Preparations in Contemporary Prosthodontics
Tooth preparations without a defined finish line have been termed in several different ways, such as knife edge, feather edge, or shoulderless. From a biological standpoint, preserving a maximum amount of sound tooth structure, as it is done in vertical preparations, might also offer a more conservative alternative to a horizontal margin design in other clinical conditions such as endodontically treated teeth, vital teeth in young individual, teeth affected by caries at the cervical third. Vertical margins on zirconia crowns have been tested in vitro and clinical reports have been published. A re-evaluation of possible advantages and shortcomings of vertical preparation design in contemporary prosthodontics will be presented in light of recently published literature.

Enhancing Esthetics with a Fixed Prosthesis Utilizing an Innovative Pontic Design and Periodontal Plastic Surgery
This video addresses how to reestablish or maintain papilla height and the facial gingival tissue between a single or multiple missing teeth adjacent to a natural tooth or an implant by using an innovative pontic design termed the E-pontic. The anatomical shape of the E-pontic will predictably support and maintain the facial gingival margin and interdental papillae over an acceptable alveolar ridge. The E-pontic design fulfills the prerequisites for maintaining a healthy periodontium and its ability to achieve a predictable long-term esthetic and functional result. The emergence profile of this pontic design that is similar to that of a natural tooth ensures that no palatal gap forms which could potentially cause phonetic problems. Trapping of food particles does not occur because of its unique adaptive design to the pontic recipient site.

Can We Still SAVE Teeth? Modern Periodontal Surgical Concepts for the Natural Dentition
Biologic shaping is an adjunct to conventional crown lengthening. Conventional crown lengthening uses the existing margins of an old restoration or the cementoenamel junction (CEJ) of a non-restored tooth to determine the amount of bone removal necessary to establish adequate space for the biologic width. Creating proper space for the biologic width ensures that a new margin will not impinge upon the periodontal complex and reduces the likelihood of future inflammation.

This gives the practitioner total control over the case, gives the patient a true visualization of what the permanent teeth will look like and gives the laboratory an accurate tooth position to be copied into final form. The d-APA is done with flowable compostite in the patient’s mouth thru a series of additions and reductions to the teeth to gain final position. Modern Occlusal Concepts in Esthetic & Implant Dentistry Premium Member Content

Modern Occlusal Concepts in Esthetic & Implant Dentistry
Occlusion is probably not one of the most popular topics in dental meetings and conferences nowadays especially if compared to implants and esthetics. Indeed it was considered in dentistry much more important just a few years ago than it is now. There is growing distance between the evolution of scientific research in the field, including available recent literature and several popular clinical approaches. It seems even difficult to have universal definitions related to this topic: the term “occlusion” itself is often associated with different adjectives to define it and many other terms are used with quite often differences in meaning, each of us gives a different meaning to the same terms, according to differences in treatment philosophies or in different specialties.

Esthetic and Functional Rehabilitation: A Case Report
Most people want to look beautiful. The ability to express feelings is fundamental to the quality of life, but expressing emotions is impossible without the freedom to smile. A patient with an ill-fitting removable denture and a fixed, porcelain-fused-to-metal bridge sought care to improve her smile. This fixed bridge was removed and direct provisional restorations placed, followed by crown-lengthening surgery and implant placement, waiting 10 months for the patient to heal. We waited one month to check occlusal stability and patient satisfaction and then made the final precision impressions and master casts. We cemented the final restorations; lithium disilicate allowed the creation of an esthetic result without sacrificing strength with use of both full-anatomy and layered techniques. Dental implants make it possible to have non-removable teeth that can be integrated esthetically and functionally, allowing the recreation of tooth color, form, and structure to reestablish a pat

Moreover, it is essential that all the team members participate proactively in the treatment planning process in order to ensure the best possible result for the benefit of the patient. Consequently, consistent interaction among the members of the dental team is required. Technicians should have a basic understanding of clinical procedures and the development of target-oriented communication protocols are essential prerequisites for a smooth workflow and the provision of high-quality service. Dentists should be aware that crucial treatment steps such as the diagnostic wax-up, the fabrication of the mock-up, shade selection, material selection, prep design, and the fabrication of temporary restorations, etc. can be greatly improved by taking into account the technician’s perspective.

Treatment of the Fully Edentulous Patient: Prosthetic Implant Solutions
Restoration of the completely edentulous patient with dental implant therapy is a rewarding treatment modality for the patient and dentist. In order to accomplish consistent and predictable treatment outcomes, an understanding of the fundamental principles of implant biomechanics, prosthetic, surgical and treatment planning considerations is required. Dr. Aldo Leopardi’s objective is to bring all of these factors into perspective, through an understanding of the current literature and how to apply it to clinical practice. Discussion will include, but not be limited to: Review of treatment planning for the overdenture patient. To understand the biomechanical principles in Prosthetic. Design: stress broken verses rigid design concepts. Discussion of fixed implant restorative solutions for the completely edentulous patient.

A Restorative Solution to Correcting a Geminated Central Incisor
A geminated tooth occurs when a single tooth bud tries to divide into two teeth, but the division is incomplete.  This results in a much larger tooth so when it occurs in a central incisor it can become a difficult esthetic problem. This program illustrates a restorative compromise solution to the problem.

Indications of In-Office Treatments: Topical Fluoride, Varnish, Foam and Gels
A study conducted in the 1990s, demonstrated a steady rate of decline of instances of coronal caries through the 1980s. This decline was attributed to an increase in oral health education regarding diet, oral hygiene and an increased awareness about the benefits of applying fluoride-containing topical treatments such as dental sealants varnishes, foams, or gels to the teeth of individuals of all ages in moderate-to-high risk groups for caries. This presentation reviews the history, efficacy, and indications. You will learn the history of all varnishes, differences in fluoride foams and gels as well as which fluorides work best for different patients and practices.

The Hidden Value of Digital Radiography for Caries Detection
Digital radiography allows the dentist to use powerful image analysis tools to extract information from the image that might otherwise be missed by visual inspection alone. Historically interproximal caries detection is done by visually inspecting the radiograph for evidence of a radiolucency produced by the decay and visually determining if the decay is deep enough that a restoration is required.
