3D Bone Ridge Augmentation; What Are The Limits?

Description:
This lecture will underline some of the critical aspects to achieve success in bone augmentation procedures is predictable basis. Moreover, the importance of digital planning to understand the anatomical challenges and anticipate the final clinical outcome will also be highlighted. One pillar of the presentation will be the introduction of new biomaterials and strategies to favor a faster surgery with less complications. The presenter will also discuss his understanding of what lay before us in terms of the next steps in a biological era of regeneration.

Ridge Split Procedure with Simultaneous Implant Placement Premium Member Content

Ridge Split Procedure with Simultaneous Implant Placement
A ridge split procedure has been suggested in literature for increasing the bucco-lingual width of the alveolar ridge prior to implant placement. The procedure can be performed in one or two surgical appointments. It is usually combined with additional augmentation utilizing a bone substitute material and a collagen membrane. In this video Dr Kher demonstrates a ridge split procedure that has been facilitated using a piezo surgical device. Following that he places 2 implants for the proposed restoration of a missing maxillary canine and first bicuspid after preparing the ossteotomy site using expansion screws. Thereafter he uses a CPS putty to graft the gap between the implants to fill all voids and achieves a satisfactory tension free closure.

Surgical Protocols for Socket Type 2 in the Aesthetic Zone
The topic of this presentation is an implant surgical procedures for a socket type 2 (socket missing buccal bone) in an aesthetic area, with emphasis on modified IVAN* technique for socket preservation. IVAN* stands for inter-positional vascularised augmentation neogenesis. Author`s guidelines for selection of appropriate surgical protocols for sockets missing buccal bone wall, protocols that are proved to be most effective, are presented. Early implant placement with GBR, prosthetic socket preservation and socket augmentation with socket sealing are shortly presented as alternatives for some cases. From author modified IVAN technique is shown step by step.

From Oral to Facial:Impact of Oral Surgery on Facial Esthetics - Part 1 of 2
Clinical cases of moderate to severe edentulous maxillary atrophy are described: A combination of sub-nasal procedure, sinus elevation procedure and intra-oral autogenous bone blocks were used for ridge augmentation simultaneously with dental implant placement combined with scaffold mixed with platelets-rich-plasma (PRP) or bone-marrow aspirate (BMA) and covered with platelets-poor-plasma (PPP) as a biological membrane. The correction of acquired class III, is achieved in Phase B, via Le Fort I maxillary osteotomy.

By scientific studies produced in recent years has been confirmed, in selected cases, the validity of the post-extraction implants. The use of this method implies a lower emotional impact, a single surgery and a healing often better for the patient. We will be discussed traumatic extraction methods, perfect insertion of the implants with or without bone regeneration with bone substitutes and membranes and the opportunity to combine an advanced method as immediate implant with the execution of an immediate loading to restore aesthetics in more complex cases.

Esthetic Management for Compromised Extraction Sites - Part 2 of 2
Missing soft tissue is the main reason for compromised results in the aesthetic zone. But since the soft tissue has to be supported by a sufficient amount of bone, our focus has also to be on bone. Due to the fact that no bone-preservation technique (at the time of tooth extraction) available today, gives us the perfect result we need, augmentation procedures are still needed. Appropriate bone and soft tissue engineering in all three dimensions enhances the aesthetic performance of long-lasting restorations that blend in well and emerge from the periimplant sulcus in harmony with the neighboring teeth.
