The Advantages of the Root Submergence Technique (RST) for Pontic Site Enhancement part 1

Description:
By maintaining the natural tooth root through the root submergence technique (RST) we are better able to preserve a much greater amount of surrounding tissue than the more commonly used socket preservation technique. RST maintains the natural attachment apparatus of the tooth in the pontic site which in turn allows for complete preservation of the alveolar bone frame and creates a more esthetic result in multiple tooth replacement cases.

I have a question on the sequence of your anterior lateral eruption case.I know you placed an implant in the #8 site for the ortho anchorage. But could you not have just placed brackets on the 6 anteriors and not place the implant until later? I would have thought that once you have the mesial bone at the ideal position on #7, placing the implant with hard and soft grafts would be more predictable. Great presentation by the way!

Yitzchak; Thanks. There is a risk of eruption of the root if there is no keratinized tissue covering it and a pontic with contact on the gingiva. As for cutting root, we want to cut it roughly 1mm above the bone and we perform this sometimes without a flap utilizing diamond burs and magnification. Hemostasis is key to allow visualization.

Maurice! brilliant idea as allways.. 2 ques. 1. isn't there any risk of that root trying to erupt in the future? 2. could you describe in a few words how you cut to bone level without raising a flap? thanks! Yitzchak

Endodontics is not necessary. Vital roots can be submerged. Perio roots should be debrided prior to submergence. If the root has endo, it should be sealed with glass ionomer. Multi-rooted teeth ok for RST. Can last many years in submerged state but no definite research although our cases have as many as 10+ years in some early cases.

both videos fantastic. my question is, what is the criteria to keep rootpieces inside the bone. do we need to do perfect endodontics? what should be the periodontal condition. because most of the teeth we extract are perio compromised. also does this apply to multi rooted teeth? how many years do these root pieces stay submerged? also what material is used to seal the coronal aspect of these pieces below the gum line.

Computer-Generated Abutment Using A Coded Healing Abutment: A Two-Year Preliminary Report
This article presents a preliminary clinical evaluation following the placement of 107 dental implants covered with computer-coded healing abutments. The healing abutment system evaluated herein provided information to specific computer software that facilitated the delivery of an anatomical definitive abutment. This system also assisted in the development of a titanium abutment whose characteristics mimicked the natural contours of the teeth being replaced. A two-year postoperative evaluation was performed to evaluate occlusion, gingival integrity, contact areas, inflammation, food impaction, porcelain integrity, radiographic condition and overall patient satisfaction. Improved tissue response and resultant aesthetics were observed.
