Agreed. Do nothing to traumatize the nerve and never attempt to retrieve cancelous bone from this harvest.
Only cortical bone.

Dumitru; You should do nothing to the nerve if exposed. Leave it alone. A collagen sponge without any pressure to the nerve is enough.

HY! If the alveolar nerv is expozed after the bone graft is removed from the ramus, do you protect the nerv in any way? A collagen sponge is enough? thank you!

Because it is a 5 wall defect, mesial, distal, occlusal, apical & medial wall it has great regenerative potential without anything added.
A blood clot forms after harvest and if maintained forms the basis for full osseous regeneration. Dr. Pikos has done much research on this topic and some of his articles are available on his Xpert page on this site.

This is a cookbook on how to manage autogenous blocks from the ramus buccal shelf. This is a video that should be watched several times to appreciate the nuances of this procedure. The P&S course in Orlando this June highlights this and most every other grafting procedure from simple to advanced.
