Biologic Strategies to Enhance Clinical and Aesthetic Success in Oral Implantology

Description:
This webinar will discuss clinical strategies for treatment of the complex implant site involving bone grafting and implant placement. Our understanding of cellular pathways, and our ability to control the wound response, expands our capacity to effect better aesthetic outcomes and compress treatment time. The reduction of the inflammatory phase of tissue regeneration speeds up osseointegration, prevents crestal bone loss, maintains papillary form, increases the density of peri-implant bone and increases tissue biotype. This enhanced tissue response allows us to predictably treat even the most complex surgical cases, compress time to final reconstruction, and to maintain these favorable tissue outcomes over an extended period of time. The use of dental implants with advanced biologic features and the incorporation autologous growth factors will be demonstrated.

It is considerably difficult to tunnel PRF membranes, especially over longer distances. It's tough to push through a subperiosteal space and even harder to pull with a suture. But we do it occasionally where it is indicated. Most often we are employing it in full thickness flaps, either over a grafted area or over healed bone to thicken the biotype.

Andoni; PRF retains leukocytes and white cells and PRGF removes them from their protocol. This seems to allow for greater substantivity and long term effect of the growth factors but PRGF believes it allows for greater post op swelling and morbidity. Dr. Salama

One of the biggest challenges that we dentists have are those cases that require a multidisciplinary approach which traditionally were managed using invasive and long term treatments. Nowadays, and depending on the case, the contemporary dentist can offer minimally invasive treatments which allow us to modify the shape of the hard and soft tissues. The combination of surgical lip repositioning with periodontal plastic surgery yields highly effective results in the treatment of gummy smiles. During patient examination it is important to establish the etiology responsible for the excessive gingival display. The combined technique can be considered as an alternative option to more invasive procedures. The prognosis is acceptable and the patient realizes that the esthetic problem has been minimized.

Antimicrobial Effect of a Single Dose of Amoxicillin on the Oral Microbiota
Amoxicillin is commonly used in oral surgery for antimicrobial prophylaxis against surgical-site infection and bacteremia because of its effect on oral streptococci. The aim of this study was to determine whether amoxicillin reaches the break-point concentrations in saliva and has any effect on the salivary microbiota, colonizing bacteria on mucosal membranes and on the gingival crevice after a single dose of amoxicillin. A single dose given as prophylaxis to prevent a surgical-site infection results in a significant reducing effect on the oral streptococcal microflora in the gingival crevice and may have an impact on bacteria spreading into tissues and the bacteria of streptococci.

The Management of Recession Midfacial to Immediately Placed Implants in the Aesthetic Zone
Immediate placement and loading protocols are the most technique sensitive and at the highest risk of complication, especially in the aesthetic zone. The peri-implant tissues undergo a resorption pattern that may see exposure of parts supporting the restoration, otherwise intended to be submerged, with extreme aesthetic compromise or even complete failure of treatment. The literature is not definitive in terms of any one treatment modality to recover such a complication. Grafting the exposure by a guided bone regeneration technique and an adjunct soft tissue augmentation could well restore an unaesthetic recession exposure. Recovery of midfacial recession by soft tissue augmentation alone may be successful, as is presented hereafter.
