Biologic Strategies to Enhance Clinical and Aesthetic Success in Oral Implantology

Description:
This webinar will discuss clinical strategies for treatment of the complex implant site involving bone grafting and implant placement. Our understanding of cellular pathways, and our ability to control the wound response, expands our capacity to effect better aesthetic outcomes and compress treatment time. The reduction of the inflammatory phase of tissue regeneration speeds up osseointegration, prevents crestal bone loss, maintains papillary form, increases the density of peri-implant bone and increases tissue biotype. This enhanced tissue response allows us to predictably treat even the most complex surgical cases, compress time to final reconstruction, and to maintain these favorable tissue outcomes over an extended period of time. The use of dental implants with advanced biologic features and the incorporation autologous growth factors will be demonstrated.

It is considerably difficult to tunnel PRF membranes, especially over longer distances. It's tough to push through a subperiosteal space and even harder to pull with a suture. But we do it occasionally where it is indicated. Most often we are employing it in full thickness flaps, either over a grafted area or over healed bone to thicken the biotype.

Lovely lecture. Did you tunnel subperiosteal to fit in the PRF. Which plane did you place it in?

Larry Meyer - (5/5/2014 10:13 PM)

Well done my friend.

Maurice Salama - (3/12/2014 8:27 AM)

Andoni; PRF retains leukocytes and white cells and PRGF removes them from their protocol. This seems to allow for greater substantivity and long term effect of the growth factors but PRGF believes it allows for greater post op swelling and morbidity. Dr. Salama

Great presentation. Just one question, what are the advantages of PRF over PRGF?? Thanks

Esthetic Tissue Reconstruction Around Implants - Part 1 of 2 Premium Member Content

Esthetic Tissue Reconstruction Around Implants - Part 1 of 2
Esthetics in implant-supported restorations is an important clinical objective in contemporary dentistry. Understanding the biological behavior of hard and soft tissues following tooth extraction is the first step to anticipate the physiological tissue remodeling and its consequences. Our objective in these 2 sequencial lectures was to provide straightforward decision trees related to several clinical scenarios observed in routine clinical practice. The 3 fundamental pillars for peri-implant excellence are: restorative-driven implant position, hard and soft tissue reconstruction using different biomaterials and grafts, and prosthetic management. Using this philosophical approach seems to provide esthetic and stable results over time irrespective of the initial clinical condition.

Big Picture Planning: A Comprehensive Surgical and Prosthetic Workflow
This seminar will highlight interdisciplinary aspects of treatment to achieve optimum outcomes in challenging cases. The cases presented range from single tooth replacement in the esthetic zone to full-arch rehabilitation and reconstruction of composite defects with restorative and surgical modalities. Emphasis will be placed on meeting and exceeding patient needs and working as a team in an everyday private practice setting. Surgical modalities presented will include creation of optimum hard and soft tissue architecture as well as peri-implant tissue modification procedures prior, during and after implantation. Diagnostic considerations for prosthetic rehabilitation of failing dentition and composite defects will also be discussed through case presentations.

Modern Crown Lengthening: Classification and Clinical Application
Crown lengthening procedures are indicated to provide adequate tooth structure in case of subgingival tooth fracture or caries, uneven gingival level, un-esthetic short crowns due to the tooth wear, inadequate axial height, altered passive eruption ,forced eruption of a single or multiple teeth and finally in case of gingival smile. The treatment modality for esthetic crown lengthening procedure should be based on detail diagnosis in each case because of the multifactorial etiology and because of the type of therapy selected by the clinician, will have a direct implication for the esthetic result.

A Complete 3D Strategy to Esthetic Cross Arch Restorations: Guide Surgery & Soft Tissue Interface Management with CAD-CAM Restorations
I will highlight the benefits achievable through the use of guided planning, guided laboratory procedures and guided surgeries, while maintaining the fundamental principles of osseointegration. My ten years of scientifically proven experience and clinical follow-up in guided surgery and CAD/CAM biomimetic restorations will be shared with the audience to support clinical protocols as well as practical tips and tricks with case studies demonstrating each clinical scenario. New surgical and prosthetic improvements and novel proof-of-concept techniques to enhance the natural soft tissue integration will be presented. Viewers will also broaden their knowledge of hard and soft tissue surgical management through the guided surgery, discovering the latest implant designs and prosthetic interfaces designed to achieve an ideal soft tissue seal and bone level over time.

The Use of PerioDerm for Root Coverage and Correction of Insufficiently Attached Gingiva
The objective of this article is to demonstrate the benefits of an acellular dermal matrix called PerioDermâ„¢ Acellular Dermis in correcting recession and adding connective tissue during periodontal-restorative comprehensive treatment. 1,2 It has long been established that connective tissue is an integral factor in the protection of the underlying periodontal foundation. Connective tissue tends to be tenacious in makeup and have far less vascularization than mucosa. It prevents bacterial infiltration to the underlying supportive periodontal tissues, and in restorative applications, it adds stability to gingival areas during impression taking and cementation of restorations. Though the quality of tissue is critical, it is also important that the tissue is bound down to either tooth surface or bone to serve as a supportive mechanism for protection of the periodontium.

Esthetic Correction of Gingival Recession Using a Modified Tunnel Technique and an Acellular Dermal Connective Tissue Allograft

Esthetic Correction of Gingival Recession Using a Modified Tunnel Technique and an Acellular Dermal Connective Tissue Allograft
Esthetic correction of gingival recession is an important goal of periodontal therapy. This article describes a surgical technique that combines a modified tunnel technique and an acellular dermal connective tissue allograft. With the aid of vertical incisions, a tunnel is created under the buccal mucosa of the affected tooth. These incisions enable easy access for graft placement and create mobility for gingival coronal positioning. The use of an accelular dermal connective tissue allograft eliminates the need for a surgical palatal donor site. This minimizes post-surgical complications.

Fundamentally Changing Soft Tissue Grafting

Fundamentally Changing Soft Tissue Grafting
Soft tissue augmentation procedures to increase the existing zone or create a new zone of keratinized tissue have been classically, and are routinely, performed using the patient's palate as the donor material. This article describes the simplistic use of a periodontal material that can be used for soft tissue augmentation procedures without using the patient's own palate to procure the donor tissue. This periodontal material is an acellular dermal allograft that has been used extensively in medicine.…
