poor presentation. many details were neglected.

Neil Berman - (9/4/2012 8:59 AM)

could you explain the deep mucosal-palatal suture for primary closure of the graft ? When is it placed and in what direction? Thanks

Neil Berman - (9/4/2012 8:49 AM)

good explanation of the technique

Are you routinely using PDGF over a membrane? Have you seen noticeable difference in bone growth with vs without it?

Dr. Bernstein,

I use PRP or PDGF only in very demanding esthetic cases with poor tissue quality. With Regenform I don't often use a membrane at all due to it's density and inhibition of tissue invasion.

Dr. Tischler, thanks for the excellent presentation. I've been looking for the carbides for decorticating the bone. can you please provide me with the brand and bur number. I've been looking for over a year. Thank you in advance

Dr Al Fahad,
I use a 1mmx6mm pointed bur or a very small round casrbide. Sorry for the delay.

The mIVAN Approach for Preservation Surgery
mIVAN* Technique is a nice abbreviation for complicated terminus: “ Modified Interpositional Augmentation Neogenesis”. It is basically simultaneous hard and soft tissue augmentation utilizing pediculated connective tissue graft. It works for Socket type 2 and 3 and ridges with horizontal atrophy. Combined with Partial Extraction Therapy, mIVAN technique is a nice treatment choice for the teeth with recessions.

Advances in Bone Augmentation and Related Armamentarium
Bone augmentation is one of the most important and most difficult steps in implantology. Also the standard techniques used are very sensitive, bound to a certain experience from a large number of surgeries and have a high learning curve. Despite these facts augmentation procedures are often the least planned part of the therapy and based on poor diagnostics. Due to a lack of standardized protocols these procedures are often performed by the surgeons favorite method instead of the most indicated one. It is astonishing , that for the implant placement most advanced techniques are used - cone beam, navigation templates etc. - whereas augmentation procedures are often based on a high percentage of improvisation.

Immediate Implants: Partial / Full Arch Rehabilitation & GBR - Part 2
By scientific studies produced in recent years has been confirmed, in selected cases, the validity of the post-extraction implants. The use of this method implies a lower emotional impact, a single surgery and a healing often better for the patient. We will be discussed traumatic extraction methods, perfect insertion of the implants with or without bone regeneration with bone substitutes and membranes and the opportunity to combine an advanced method as immediate implant with the execution of an immediate loading to restore aesthetics in more complex cases.

Combination Therapies in the Esthetic Zone: Steps for Success
This presentation will cover the critical aspects to consider for deciding the sequence and implementation of combination therapies. Successful esthetic zone treatment must begin with a well thought-out space management plan. This plan involves a three dimensional adjustment of the periodontal attachment, with a possibility of root submergence and partial extraction technique. Following this, a decision is made concerning the timing and position of implant placement, and an adequate hard tissue foundation can be restored using GBR with titanium mesh and collagen membrane. On top of this base a more efficient soft tissue augmentation and adjustment completes the requirements for success in the esthetic zone.

Influence of Thin Mucosal Tissues on Crestal Bone Stability Around Implants With Platform Switching: A 1-year Pilot Study
The aim of this pilot study was to determine what effect thin mucosal tissues can have on crestal bone stability around implants with platform switching. Within the limitations of this pilot study it can be concluded that implants with platform switching did not preserve crestal bone better in comparison with implants with traditional implant-abutment connection if, at the time of implant placement, thin mucosal tissues were present.
