Cad Cam Technologies to Restore a Case on Natural Dentition & Implant
Description:
In prosthetic rehabilitations, we use for many years codified and widely described in literature, with results that meet our expectations and well above those of our patients. It is true that the advent of new technologies has enabled us to use new material and new equipment that facilitate the production of an adaptation and an accuracy that has, up to now, been difficult to obtain. During this lecture, we will analyze a complex restorative cases, highlighting the operational differences that we have developed over time, with the increasing knowledge of new materials. Their use has allowed us to modify certain operational techniques and shortening lead times, while maintaining an accuracy that is more than satisfactory. The advantages and disadvantages of the methods outlined will be evaluated and analyzed on the basis of existing literature to date and on that personal experience, highlighting the limitations that both situations may arise. We'll see then how we have replaced traditional materials and techniques with other innovative new technologies that only you can work, and how the results obtained with these are systematic and overlapping times better than traditional one.
To achieve “Esthetic perfection”, every minimal detail should be considered, starting from the temporary on the day of the surgery: Dr Agnini will describe various clinical options and techniques for efficiently and esthetically fabricating a temporary restoration immediately after implant placement in the replacement of a single tooth.

Hi Mihaela, thank You for Your comment and question.
The holes in the transfer plate help us to check if the transfer plate is properly placed on the mucosa. It is critical the correct placement of the transfer plate in this technique. The anchorage is obtained by sitting the transfer plate on the tuberosity in the maxilla and on the retromolar trigon in the mandible. Whenever is possible we try to have an additional anchorage by using one / two teeth abutments and then removing them. If You need any other info just send us an email to alleagni@libero.it. Could you please comment on the surgical stent that was fabricated with the small round holes? What was the role of the holes in the acrylic template, was it for anchorage?
If yes how was it fixated during the surgery?

Caro Nicola, ti ringrazio per il tuo commento.
Il paziente quando portava lo scheletrato probabilmente aveva dei rapporti fra le arcate sicuramente migliori, peggiorati nel tempo senza il supporto posteriore.
L’analisi funzionale intraorale della Relazione Centrica e l’analisi dei modelli montati in articolatore con la ceratura di analisi associati alla corretta distribuzione dei 6 impianti, mi hanno fatto decidere per una soluzione fissa avvitata a carico immediato con leggera protezione anteriore per evitare eventuali interferenze posteriori senza, a mio parere, dovermi accollare particolari rischi.

Root Fracture During Insertion of Prefabricated Posts
The consequence of inappropriate post selection and/ or hasty insertion can result in tooth fracture and/or loss. It has been demonstrated that conical or parallel sided post design has a direct effect on root fracture during post insertion. However, during experimentation it became apparent that root size and its relationship to post size had a direct effect on root fracture. This article correlates root size, post size, and fracture rate during insertion of prefabricated posts.
