Consecutive Anterior Implants: Surgical and Restorative Management of Esthetic Failures - Part 4

Description:
Esthetically successful placement of consecutive implants in the anterior region, especially in the lateral/central environment, remains as one of the most challenging endeavors in implant therapy. This webinar will focus on the diagnosis and treatment design options for the implant replacement of two or more adjacent teeth in the anterior region with emphasis on preventing mid-facial and interdental tissue loss. In addition, the surgical and restorative management and correction of esthetic failures will be discussed including the use of connective tissue allograft.

Tom, thanks for bringing us back to a very important subject matter. To answer Simon's question, in my opinion an important factor contributing to the failure of that 1st case is related to not factoring in the deficiency present at the inter proximal height of bone on the mesial of the lateral; look at the pre-implant slide, as well as intra-surgical. That deficiency should have been factored into a) factoring prognosis, b) incision design, and c) Whether to do a 1 stage approach or 2 stage. The corrective procedure performed was necessarily 2 stage and required managing a multitude of surgical and restorative considerations, as well as overcoming the odds.

Thought that the question below by Simon Milbauer was worthy of an answer.

State of the art cases and superb lecture docs!My question is what in your opinion was the reason behind the esthetic failure of the implant in your 1st case?It seemed to be a little bit close to the adjacent lateral incisor on x-ray but when flap was raised the distance appeared acceptable? Many thanks, Simon Milbauer

Samir; If you are thinking about effect on phonetics, as long as their is an air tight seal with the gingiva this has not been a problem at all. good luck Dr. Salama

Soft Tissue Recession Around Implants
When treatment with dental implants is indicated, an accurate diagnosis must be made to evaluate the clinical parameters and determine the optimal time for immediate or delayed (ie, early or late) implant placement and loading following tooth extraction. It is also important to identify complications and their implications on the aesthetic outcome.1 This article explains behavior of the hard and soft tissue around the implant, evaluates the timing of implant placement after extraction, and reviews various parameters that influence tissue marginal remodeling.
