Questions & Comments
Ronald Goldstein - (12/19/2010 6:09 PM)

Cyndi,

If the stains are in the incisal thirds, depending on the color of the stain, you might consider enamel micro-abrasion...certainly the white hypocalcification stains can many times be successfully treated this way. Otherwise I would consider preparing the tooth with an AC10 football shaped diamond(BrasselerUSA) to take as much of the stain out and then bond either using an opaquer first or an opaque composite such as one of the opaque Venus Diamond (Heraeus) shades that are quite good at masking stains even without additional opaquer. The MI paste is for desensitization so if the patient needs it fine...otherwise the desensitization from the composite bonding technique will most times take care of any sensitivity.

Thank you for the great tips , what if the stains are only int he incisal thirds of the 2 centrals, would you advise me to use the MI paste or should i go with masking it with composite?

Ronald Goldstein - (11/3/2010 5:43 PM)

Yes, fluoride can be effective but MI Paste (GC) is more effective. And if you use a bleaching agent that has a desentization agent in it then there should be much less sensitivity.

thanks for the lecture..so useful :).. I have question, How we can overcome the sensitivity problem after the bleaching..Is applying flouride is effective ?? Any other ways?

HI DOC,GREAT PRESENTATION AND GOOD LECTURE....MY QUESTION IS,HOW SENSITIVE ARE TEETH TO THESE BLEACHING TECHNIQUES.POST OP?COZ I AM PRACTISING IN A COLD CLIMATE.?WHAT INSTRUCTIONS DO WE ADVICE REGARDING FOOD AND ANY SPECIAL CARE ?TO AVOID DISCOLORATION.

David Garber - (9/3/2010 8:35 AM)

Hi Joao we find that Ultradent Opalescence Endo works very well as an internal Bleach product ."walking Bleach" - - it is a 35% HYDROGEN PEROXIDE as compared to carbamide peroxide , and so is effectively much stronger - - it is also Catalysed by adding a Heat source such as a bleaching wand - - there is little research data to indicate that light/heat accelerates traditional bleaching but clinically it does look brighter/ lighter possibly because of dessication. . .also may induce a little more sensitivity anyone else have similar experience ? David

Hi Dr Goldstein! I tried all kind of products to bleech and i can say that opalescense always got funtastic. After that i recommend a toothpaste that helps the remineralization as nutri-émail does very well. Whenever i use light bleeching i iam not so happy. What is the best product to do endo bleech? thanks for your lecture. hug

At-Home Bleaching: A 25-Year Retrospective
The concept of at-home bleaching self-administered by the patient and supervised by the dentist became widely known in the profession about 25 years. At the time, little was known about bleaching except that application of a peroxide-based gel to the teeth could substantially lighten them in 4-6 weeks. Since then, a large volume of laboratory and clinical research have greatly expanded and enhanced our understanding of the process. This presentation provides a brief summary of what we have learned over the last 25 years, including how bleaching works and how long it lasts, safety issues, side effects such as tooth sensitivity, effects on teeth and materials, extended uses for bleaching, and over-the-counter options.

The Status of Tooth Whitening Techniques 1999
Tooth whitening of vital teeth continues to have a major impact on the practice of dentistry. The growing public interest in having whiter, brighter teeth is clearly evident in the advertisements from toothpaste manufacturers on "whitening" formulations of their products and by the number of individuals seeking whitening procedures from their dentists. In addition, new over-the-counter whitening products continue to emerge in a marketplace that cannot seem to get teeth white enough, bright enough, fast enough.
