Immediate Extraction, Implant Placement and Provisionalization in one visit

Description:
In this 2nd of a series on immediate loading, Dr. Henry Salama describes the criteria necessary for successfully utilizing minimally invasive protocols within the esthetic zone. In addition, Dr. Salama demonstrates various clinical options and techniques for efficiently and esthetically fabricating a temporary restoration immediately after implant placement in the replacement of a single tooth. Virtual treatment planning and CAD-CAM technology are also discussed.

Beautiful work! Infomation of outmost importance about the submergence profile.

Thomas Pomierski - (10/28/2009 10:09 AM)

I wish you would have showed the connective tissue graft as that is imperative to the final results?

henry salama - (10/27/2009 11:43 AM)

To Drs Kim and West,
I usually wait approximately 8-10 weeks in the maxilla and 4-6 weeks in the mandible before restoring with the final crown.
As to the abutment impression coping shown in the video for the Xive implant used, that was done utilizing Cercon/Denstsply technology and was fabricated by my private lab. However, I believe any lab equipped with CAD/CAM technology can fabricate such a coping on demand. In addition, there are abutment systems such as the Zimmer HLA contoured abutments that have dedicated snap-on components readily available for them in both straight and angulated configurations. In such cases, a customized ceramic coping is not necessary.
Also, as Dr. Scott Ganz mentioned, his articles and lectures available on Dentalxp are a great reference for further follow-up on this subject as well as others related to CT-Derived Model-Based surgery and I highly recommend them.

Great technique... Expanding the scope of the technology and connecting planning with execution. You might want viewers to download original related articles defining the beginning of this technology right here on DentalXP... "Use of Stereolithographic Models as Diagnostic & Restorative Aids for Predictable Immediate Loading of Implants" from my article published in 2003... a follow-up article is also posted... "CT-Derived Model-Based Surgery for Immediate Loading of Maxillary Anterior Implants" from 2007... available for download on this site... and videos under Dr. Scott Ganz.

Great video, do you have a specific lab or company that fabricated the abutment/impression coping/crown coping? Also, did you currette out the defect from the root fracture/ graft that area. thanks.

Daniel Kim647 - (10/23/2009 10:40 PM)

I would like to know how long you waited before restoring it with the permanment crown.

CT-Derived Model-Based Surgery for Immediate Loading of Maxillary Anterior Implants
A novel approach to predict implant placement utilizing CT-derived model-based surgery was performed for a maxillary-anterior aesthetic immediate extraction/immediate restoration; root proximity and inter-implant positioning was an additional complication. The use of state-of-the-art diagnostic and treatmentplanning tools described in this article has been shown to be effective for both partial and completely edentulous patient presentations. These tools can serve as a foundation for accurate, highly practical, and repeatable CT-scan–derived modelbased presurgical prosthetic planning for the immediate loading of dental implants.

Instrumentation for Modern Day Implant Surgery

Instrumentation for Modern Day Implant Surgery
Public awareness of the aesthetic possibilities through the replacement of missing or nonsalvageable teeth has been elevated with advances in dental implant therapy and related hard and soft tissue regenerative therapies. This has led to an increase in the fabrication of fixed prostheses supported by either natural teeth, dental implants, or removable prostheses anchored by implants rather than conventional tissue- or tooth-supported partial dentures.
