Description:
The management of the hopeless tooth or an edentulous span in the Estheitc Zone is amongst the most clinically demanding procedures in tooth replacement do to the demands of tissue preservation and patient management during the healing phase. As important as the implant surgical process in these areas is the role of the immediate provisional resotration. Tissue support, emergence profile and The provisional restoration phase of implant therapy has become one of the most critical steps in immediate or delayed placement for several reasons:
•Patient comfort and decreased number of clinical procedures required.
•Survival rates comparable to delayed placement.
•The potential for tissue preservation through soft tissue adherence to the provisional restoration surface.
A step-by-step protocol will be presented in how to fabricate the proper screw-retained provisional restoration from the fabrication of the acrylic shell to managing and capturing the proper supportive subgingival contour.

Excellent well thought out presentation - I enjoyed it immensely. I have two questions for you :
you showed some beautiful outcomes. However, the key issue is what do they look like in the long term ? I have had the fortune of creating some excellent outcomes with flapless immediate/provisionalisation as well only to review them some four years later to see the buccal/lingual dimensions continue to remodel and ultimately resorb - revealing a buccal concavity.. Have you seen this with the cases you have shown ?
secondly, you state that 'perfect' outcomes only occurred with 66% of the cases with your technique. presumably, the 33% group presented with 'early' bucco-lingual remodelling and early buccal concavity ?
surely, the thinnest of intact labial plates are in this category ?
your thoughts on this would be much appraciated

Stephen,Excellent presentation and keen observations. I am presently in what you would call a non bleeder at 3 months. I your opinion, can a non bleeder be transformed to a bleeder or is it too late?
