Immediate Molar Implant Placement With Superwide Diameter Implant - Part 1 of 2

Description:
Salvation of failing molars are now a clinical dilemma due to cost and predictability. Therefore most dental clinicians prefer extraction, healing, than placing of the implant. However, this often involves socket preservation and delayed approach due to technical difficulties in placement of an implant at the septum of the molar extraction site. Dr. Lee will present a new techique utilizing a trephine to collect autogenous bone, and placement of a superwide implant 6-8mm in diameter to fill up the extraction site.

Mansour; I also do not utilize a membrane at time of immediate implant placement. As long as the gap is less than 2.25mm to the surrounding walls the literature does not support the need for a membrane. Dr. Salama

nice presentation,but you did not cover your allograft with any kind of membrane , don't you think ,the bone quality around the implant will not be the best,since you did not cover it with membrane I mean in cases ,where you don't have enough auto-genes bone and you are forced to use allograft?thanks again

Great job Doctor Lee. As an orthodontist just learning how to place implants you have opened my eyes to several ways to better serve my patients. The majority of my orthodontic colleagues limit their practices to traditional orthodontics. After 40 years of traditional ortho I am ready to used learned ortho skills to more favorably plan and place implants. Thank you for your contribution.

Controlled Ridge Splitting (CRS)
To demonstrate a new technique of controlled ridge splitting (CRS) in severely atrophied maxillary cases as an alternative to autogenous block graft. Twenty cases were completed using a controlled ridge splitting (CRS) technique with a total of 65 implants were placed in severely atrophied Maxillae and followed after the implants were loaded. Results: The CRS technique was used in very complex cases, where the alternative method will be autogenous block graft. A total of 65 tapered implants were placed. The implants diameter ranged between 3.25-5mm with a length ranged between 10-13mm. The implants were restored and were followed for 1-3 years. All implants achieved osseointegration and continue to have successful prostheses. Conclusion: The CRS is a predictable treatment option and could be a good alternative to autogenous block grafts is severely atrophic maxillae.

Bone Reconstruction: A New Algorithm for the Implantologist
Bone augmentation is often required to place an adequate number of dental implants in ideal positions for prosthetic support. In addition to biomechanical demands bone augmentation can also provide proper ridge contour for improved esthetic outcomes. Disadvantages of bone augmentation techniques include increase morbidity, surgical time, costs and treatment length. As such a trend has develop towards treatment approaches that avoid bone grafting including shorter and narrower implants, angled implants and/or fewer implants for prosthetic support. A discussion will include the use of defect topography as a guide in choosing a suitable augmentation technique. Several considerations that allow for tension free wound closure will be reviewed.
