Internal Hydraulic Sinus Lift Procedure utilizing the Crestal Approach

Description:
In this video case report, “conservative” internal management of posterior maxilla is highlighted featuring new tools, techniques and biologics. Evaluation and Management of the deficient posterior region is handled by via CBCT Diagnostics and the CAS-KIT Crestal Approach Sinus Kit from Hiossen. Flap Reflection, Drilling sequence, Hydraulic Sinus Lifting of the Schniderian Membrane as well as the utilization of PRGF, Fibrin and Synthetic Bone Putty is highlighted in a conservative crestal approach. This is followed by final suturing using a horizontal and running mattress method.

2cc water for 10mm lift. Typically only 0.5gm bone for crestal lift of 3-5mm and 2.0-4.0gms for lateral approach. Dr. Salama

Can you please tell me what type of handpiece is used with the Neobiotech SLA kit. It has to be a surgical handpiece (1:2.7) but it also has to be latch type. Please advise

Dimitris; Thank you. Yes, if you see the video, I mention how the fluid comes back out of the crestal access. That is another way but with Valsalva I place a mirror over the access and if it becomes "fogged" then there is air escaping. Hope this clarifies things. Dr. Salama

Thank you for the wonderful presentation Dr Salama. Isn't the Valsava test contraindicated for Crestal approach sinus lift because if there is not a perforation the membrane expands again on the sinus floor?Dr Mazor recommended the saline irrigation test to check for perforation. Is there any other safe way to check it?

Mohammad and Darawsha; Thank you for your kind words. Regarding test for sinus perforation you must perform the Valsalva test. The Valsalva maneuver is performed by moderately forceful attempted exhalation against a closed airway, usually done by closing one's mouth, pinching one's nose shut while pressing out as if blowing up a balloon. If there is a large perforation I would access the sinus again laterally and seal the perforation with a collagen membrane and Fibrin. Typically I utilize 1-2gms of bone per side but it depends on the number of teeth involved. regards Dr. Salama

Sinus Floor Augmentation With Simultaneous Implant Placement Using Choukroun’s Platelet-Rich Fibrin as the Sole Grafting Material: A Radiologic and Histologic Study at 6 Months
Sinus augmentation with simultaneous implant placement without bone graft material is a hotly debated technique. This technique could be improved and secured by the use of an autologous leukocyte-and platelet-rich fibrin (PRF) (Choukroun’s technique) concentrate. The objectives of this study were to assess the relevance of PRF clots and membranes as the sole filling material during a lateral sinus lift with immediate implantation using radiologic and histologic analyses in a case series.
