Managing the Furcation in Periodontal and Restorative Therapy

Description:
In this Presentation, Dr. Daniel Melker describes, in great detail, the management of molar furcation involvement in preparation for crown and bridge therapy. Principles of crown lengthening and periodontal-prosthesis are reviewed and illustrated to guide clinicians in identifying critical factors necessary for making successful surgical and restorative decisions to overcome the challenges of furcation management.

Questions & Comments
Daniel Melker - (12/27/2012 1:35 PM)

Bassam, you are correct BS is a way to spare the bone where possible. It is only used when doing perio-restorative where there are cores and provisionals. Now every now and then I will shape a furcation without a crown to be placed. I will send you the core info shortly.

Bassam Algheryafi - (12/24/2012 5:32 PM)

Dr.Melker, thanks for such well-illustrated presentation. My understanding of the biologic-shaping is to spare the bone, right? would you perform this procedure on all molars that need crowns/bridges or just those with obvious/doubtful furcation involvement? Also, I want my present for this year Christmas. Can you please send me cases on how you do on cores? Thanks. Bassamg1981@gmail.com

Band Ditto64 - (8/24/2012 8:31 AM) Tremendous and useful everyday dentistry. What a gift to be able to interact with the creators of these videos directly thru DentaXP. No one else offers this medium online. Dr. Melker, due all cases end with a long feather edge margin? If we prepare beyond the margin does this leave a "roughened" root surface that can attract plaque? Is this an issue for you in long term maintenance. Again thank you and DentalXP. Thanks Band.- Band, I have been doing this procedure since 1975 and really have not modified it a great deal except burs. They must be diamonds and end cutting. Minimal taper to avoid decreasing the occlusal table. Because the object is to create a perfect interface between bone and tooth surface with MINIMAL TAPER a new margin really can be placed anywhere as the tooth surface is smooth. We do recommend supragingival margins but frankly if you go into the sulcus it should not matter. I find plaque accumulation only to be an issue if the margins are rough. That is why I love supragingival margins because we can pretty perfect margins and thus less plaque accumulation. Hope this answers your question! Thanks

Managing the Extraction Site - Optimal Preservation and Enhancement Techniques
There is rarely a day that clinicians do not have to deal with an extraction site. If implant therapy is a consideration, then bone and soft tissue quantity and quality must be factored into treatment options. The ideal opportunity for bone preservation exists at the time of tooth extraction. All too often this opportunity is lost. Extraction techniques will be demonstrated that not only preserve but augment the amount of alveolar bone. The importance of piezosurgery, atraumatic surgical technique, the use of bone grafts, resorbable membranes and growth factors will be elucidated along with soft tissue enhancement.

Smoking - A Risk Factor for Periodontal Disease (in Spanish)
Tobacco is a major risk factor in the development and severity of periodontal disease, as well as in the response to periodontal therapy.
