Bassam, you are correct BS is a way to spare the bone where possible. It is only used when doing perio-restorative where there are cores and provisionals. Now every now and then I will shape a furcation without a crown to be placed. I will send you the core info shortly.

Dr.Melker, thanks for such well-illustrated presentation. My understanding of the biologic-shaping is to spare the bone, right? would you perform this procedure on all molars that need crowns/bridges or just those with obvious/doubtful furcation involvement? Tremendous and useful everyday dentistry. What a gift to be able to interact with the creators of these videos directly thru DentaXP. No one else offers this medium online. Dr. Melker, due all cases end with a long feather edge margin? If we prepare beyond the margin does this leave a "roughened" root surface that can attract plaque? Is this an issue for you in long term maintenance. Again thank you and DentalXP. Thanks Band.-

I have been doing this procedure since 1975 and really have not modified it a great deal except burs. They must be diamonds and end cutting. Minimal taper to avoid decreasing the occlusal table. Because the object is to create a perfect interface between bone and tooth surface with MINIMAL TAPER a new margin really can be placed anywhere as the tooth surface is smooth. We do recommend supragingival margins but frankly if you go into the sulcus it should not matter. I find plaque accumulation only to be an issue if the margins are rough. That is why I love supragingival margins because we can pretty perfect margins and thus less plaque accumulation. Hope this answers your question!
