Maxillary Sinus Augmentation: Complications, Prevention and Future

Description:
Sinus augmentation using variable bone substitutes has been known as predictable method to reconstruct the pneumatized sinus cavity for several decades. However various complications such as bacterial and fungal infection, oroantral fistula and failure of bone regeneration have been reported after bone graft was done in the sinus. Numerous studies reported successful bone formation and osseointegration in cases of sinus membrane elevation without bone graft. No bone added sinus augmentation is free from postoperative infection and the risk of cross-contamination according to studies. Only autologous bone reformation is achieved when bone is not graft in the sinus. This presentation will demonstrate new bone formation in the sinus without bone graft using lateral window technique and crestal approach - hydrodynamic piezoelectric internal sinus elevation (HPISE) - with clinical, radiographic and histologic evidence from animal and human. Dear Dr Gala, 1. Bone graft doesnt improve implant stabilty 2) What piezo tips did you use for HPISE technique? HPISE TIP SET were used 3) Would you prefer to have two bony windows if there is a sinus septum? I prefer one large window..thank you Ds

Dear Dr. Singh , hpise tip is compatible with Italian piezo such as surgrybone or piezosurgery, not piezotome.Pleases visit hpise.kr

Sticky bone is mixture of autologous fibrin glue and articuate bone powder. It doesn't migrate during healing period and accellerate healing thanks to growth factors.

Transcrestal Sinus Floor Elevation: Most Frequently Asked Questions
Over 20 years of clinical application and research on transcrestal sinus floor elevation (TSFE) procedures using a variety of implants, graft materials and instrumentation has proven its reliability, and versatility. While improving upon the original osteotome technique they have not yet eliminated the need for a more invasive lateral approach, especially at sites with very limited (1-2 mm) sub-antral bone. Extensive analysis of the existing research and long-term clinical experience will provide the answers to the most pertinent questions surrounding the transcrestal approach including but not limited to: patient selection, clinical limitations, instrumentation, graft selection/necessity and management of membrane perforations.

Sinus Augmentation for Single-Tooth Replacement in the Posterior Maxilla: A 3-Year Follow-up Clinical Report
A technique for single-tooth sinus lift and simultaneous implant placement in the posterior maxilla is presented. Ten hydroxyapatite-coated cylindric implants, 13 to 15 mm in length, were placed together with a composite bone graft of demineralized freeze-dried bone allograft and autogenous bone in 10 adults. Surgical technique and anatomic considerations are discussed. Follow-up of 3 years showed successful function and no cervical bone loss in all patients.

The lateral window approach to maxillary sinus augmentation is a well-accepted treatment option in implant dentistry. The most frequent complication reported with traditional techniques has been the perforation of the Schneiderian membrane, with erforation rates ranging from 11% to 56%. The purpose of this retrospective, consecutive case series from two private practices was to report on the rate of Schneiderian membrane perforations and arterial lacerations when a piezoelectric surgical unit was used in conjunction with hand instrumentation to perform lateral window sinus elevations.
