Maxillary Sinus Augmentation: Complications, Prevention and Future

Description:
Sinus augmentation using variable bone substitutes has been known as predictable method to reconstruct the pneumatized sinus cavity for several decades. However various complications such as bacterial and fungal infection, oroantral fistula and failure of bone regeneration have been reported after bone graft was done in the sinus. Numerous studies reported successful bone formation and osseointegration in cases of sinus membrane elevation without bone graft. No bone added sinus augmentation is free from postoperative infection and the risk of cross-contamination according to studies. Only autologous bone reformation is achieved when bone is not graft in the sinus. This presentation will demonstrate new bone formation in the sinus without bone graft using lateral window technique and crestal approach - hydrodynamic piezoelectric internal sinus elevation (HPISE) - with clinical, radiographic and histologic evidence from animal and human. Hi Prof, why sometimes blood come out from noose after sinus lift?, is it complication and how to handle. Thank you

Dong Sohn - (9/10/2015 6:46 AM)

Dear Dr Gala, 1. Bone graft doesnt improve implant stabilty 2) What piezo tips did you use for HPISE technique? HPISE TIP SET were used 3) Would you prefer to have two bony windows if there is a sinus septum? I prefer one large window..thank you Ds

Dong Sohn - (9/10/2015 6:43 AM)

Dear Dr. Singh , hpise tip is compatible with Italian piezo such as surgrybone or piezosurgery, not piezotome.Pleases visit hpise.kr

Very good lecture Dr D-S Sohn.. Wanted to check ask you about HPISE inserts, are they universal tips and can they be attached to PiezotomeSOLO unit(Satelec-ACTEON)?? Thank you for the lecture. Looking forward to your reply

Jigar Gala - (8/8/2015 2:57 AM)

Excellent presentation Dr.Sohn.I have some questions: 1) Would you recommend placing bone graft material after lateral elevation, if the Implant stability is less? 2) What piezo tips did you use for HPISE technique? 3) Would you prefer to have two bony windows if there is a sinus septum?

Thank you so much, Michaeal

Sticky bone is mixture of autologous fibrin glue and articuate bone powder. It doesn't migrate during healing period and accellerate healing thanks to growth factors.

What is Sticky Bone ?

Sinus Floor Elevation: A Clinical, Radiographic and Histologic Overview
This presentation and imbedded HD videos will highlight the important aspects of the Sinus Lift procedure and how to increase the success rate while minimizing complications. An overview of the procedure with detail radiographic planning and follow up outcomes will be displayed and histoligical results highlighted to gain an appreciation of the timing of bone fill and when you can place and load implants in the augmented sinus. Complications and their management will be equally covered with an impetus on case and patient management and follow up care. Common medications and techniques for each procedure will be noted.

Tissue Engineering and Platelet Derived Growth Factors: Evidence Based Therapy
Platelet derived growth factors are now routinely utilized in reconstructive therapy. This presentation describes very detailed and evidence based guidelines for clinicians interested in enhancing their abilities in tissue engineering, especially as it relates to bone augmentation. Specifically, while successful bone augmentation requires the standard surgical parameters of space maintenance, low pressure on the grafts and tension-free flap closure, optimization of this goal requires management and enhancement of the local biological conditions with growth factors. Towards that end, platelet concentrates through the constant release of growth factors are able to promote and enhance new vascularization, provide plasma protein, normal lipidemia, as well as increased collagen and fibrin activity.

Innovative New Technique for Minimally Invasive Crestal Sinus Lift Approach
This webinar will describe the protocol for a trans-crestal sinus lift by using an innovative product called the Magnetic Mallet. This technique allows the clinician to perform a sinus lift and same time osseodensification with immediate implant placement, in an atraumatic and conservative fashion. Dr Baldi will describe the benefits he has seen in his daily practice as well as the additional opportunities in treating "everyday cases" with this magnetic dynamic technology.

Sinus Augmentation for Single-Tooth Replacement in the Posterior Maxilla: A 3-Year Follow-up Clinical Report
A technique for single-tooth sinus lift and simultaneous implant placement in the posterior maxilla is presented. Ten hydroxyapatite-coated cylindric implants, 13 to 15 mm in length, were placed together with a composite bone graft of demineralized freeze-dried bone allograft and autogenous bone in 10 adults. Surgical technique and anatomic considerations are discussed. Follow-up of 3 years showed successful function and no cervical bone loss in all patients.

Alternative Applications of Guided Surgery: Precise Outlining of the Lateral Window in Antral Sinus Bone Grafting
Computed tomography (CT) and the application of CT-based guided implant surgery allow clinicians to provide enhanced precision and accuracy in implant surgery. Because of the difficulty in transferring a patient’s often complex anatomic sinus configurations, as viewed on a preoperative CT scan, into precise osteotomy cuts at antral bone graft surgery, a prototype cutting guide was developed. The surgical guide was developed through the use of CT imaging and the stereolithographic process to precisely position the lateral window, facilitating schneiderian membrane elevation. This report demonstrates the step-by-step method to perform precise guided sinus window preparation using computer software and a stereolithographically generated surgical guide.
