Jerome Bistritz, I have forwarded your question to Dr. Jorge Ravelo here is his answer. Rigid fixation is the gold standart in OMFS. He uses it for Lefort 1 osteotomies. However for genioplasties he has better 3D control of the chin position with osteosintesis wires. See my previous webinars in XP. I have well over 35 Orthognatic Surgery cases operated by Dr. Ravelo. In order for you to see the fantastic surgical results he gives us. Stable and great dentofacial esthetics with occlusal functional results. Miguel Hirschhaut.

Our oral and maxilofacial surgeon uses both systems. He decides what system is best in each case. Dr. Jorge Ravelo has well over 7000 orthognatic surgery cases done. His volume of cases and experience is simply amazing. In my humble opinion he is one of the surgeons that does and has done more orthognatic cases anywhere in the world. If you write me an email to hirschhautmiguel@yahoo.com. I can have him write you back with answers to any questions you might have for him. I have been working with him for the last 21 years. However he has at least 16 years more experience that me and has presented his cases and protocols at the AAOMFS. I am sure you have seen his lectures. Why not use micro plates instead of wire osteosynthesis I found them much easier and more stable for the Lefort and genioplasty procedures?

Thanks Dental XP for this new opportunity to present our multidisciplinary cases done in Caracas, Venezuela. The anterior esthetic zone is the greatest challenge to any clinician. The Cad/Cam Technique: Does it Really Make a Difference in Adult Esthetic Orthodontics?
Over the last couple of decades, Lingual Orthodontic treatments have dramatically risen in popularity due to the abundance of worldwide courses and conferences, the large variety of lingual brackets, the improvement in laboratory procedures, updated CAD-CAM technologies and the implementation of 30 years of experience. Yet, most clinicians still assert that the results are not predictable and not as good as seen with the buccal appliance. The reasons are multifactorial, but are mainly attributed to ignorance and the lack of knowledge of the biomechanics limitations of the orthodontic appliances and the importance of treatment planning. Various CAD-CAM techniques will be surveyed with numerous clinical cases.

Radiographic diagnosis is an important part of orthodontic treatment planning. 3D conebeam CT (CBCT) has advanced orthodontic care for many patients; however, it has not supplanted conventional two-dimensional radiographs which, when comparing equal levels of radiation, offer superior image quality. This Webinar explains when CBCT is indicated with/in place of 2D radiography and how to prescribe 2D and/or 3D x-ray on a patient-specific basis, thereby conforming to Federal guidelines mandating radiation levels 'as low as reasonably achievable' -- especially with regard to younger patients.
