Minimally Invasive Sinus Augmentation Using Ultrasonic Piezoelectric Vibration & Hydraulic Pressure

Description:
Compared to the lateral window approach, the crestal approach has the advantage of being minimally invasiveness which contributes to less postoperative discomfort. Variable crestal techniques have been reported to overcome the disadvantages of the lateral window approach. This video demonstrates hydrodynamic piezoelectric internal sinus elevation (HPISE) which was reported in 2008. Unlike conventional crestal approaches, HPISE is free from postoperative positional vertigo because ultrasonic piezoelectric vibration is used to break sinus floor directly, not using osteotome. HPISE doesn’t usually rely on bone compaction to elevate sinus mucosa, so even and gentle elevation of the sinus membrane is possible due to hydraulic pressure from internal irrigation and the rate of membrane perforation is very low. The amount of membrane elevation is compatible with lateral window technique. So HPISE is an alternative surgical method to the lateral sinus augmentation technique and can be used with or without bone graft.

Dear jerome bistritz

You are interested in follow up of Vital used in this case. Final result was good.. I don't use this one anymore due to too much flowable. Wound should be dried before applying it. Sticky bone is better. Regarding on HPISe, you can read articleson cinical studies and other videos posted at xp or facebook or www.waups.org

I am very sorry but what is the purpose of the video without the 4 to 6 month follow up with a CBCT images demonstrating the resulting osseous gain of bone volume and quality in the maxillary sinus augmentation. No follow up no proof of claim? Both for the bone putty and occlusive dressing???Please submit the follow images because the technique would a be less invasive than lateral window approach if it works

Hello Mark, the rate of membperforation is very low. Water is gentlebut powrful. This tecnique is much simpler safer than Physioloft becuse of mnimal instrumentation..Thank you,ds

Any chances of perforating membrane by the water pressure. Would you say this technique is safer than the Physiolift by Mectron?

Over 20 years of clinical application and research on transcrestal sinus floor elevation (TSFE) procedures using a variety of implants, graft materials and instrumentation has proven its reliability, and versatility. While improving upon the original osteotome technique they have not yet eliminated the need for a more invasive lateral approach, especially at sites with very limited (1-2 mm) sub-antral bone. Extensive analysis of the existing research and long-term clinical experience will provide the answers to the most pertinent questions surrounding the transcrestal approach including but not limited to: patient selection, clinical limitations, instrumentation, graft selection/necessity and management of membrane perforations.
