Occlusion in Implant Dentistry - Part 2 of 2
Description:
In this presentation Dr. Carlo Poggio delves into the biomechanics and clinical relevance of proper occlusal management in implant dentistry. Combining a thorough review of the literature with clinical dental applications, Dr. Poggio attempts to suggest a rational for designing the occlusion for implant restorations. Topics covered include comparison of occlusal management for teeth and implants. In addition, the importance of controlling the occlusal forces to protect the bone-implant interface from overload and the role of occlusion in peri-implant microbial infection.

Hello Dr Poggio, Thank you for this excellent presentation. What kind of occlusal scheme would you use for ALL-on-four acrylic or PFM prosthesis? Best, Nainesh Desai

Thanks Maurice!, as you of course know there is no evidence to support 4 vs 6 or more implants (Martin Gross review is very interesting about this). I have been treating patients both with 4 and with 6-8 implants and personally I haven't seen much differences in bone resorption pattern. In n my own experience I have seen patients with bone loss around 8 implants as well as around 4, and I have seen patients with a very stable bone level on 4 as well as on 8 implants. it would be probably difficult to assume that this differences are related to occlusion, as you are well aware there are so many different factors playing a role (implant type, implant positioning related to crest, residual bone volume, surgical management etc.). So occlusion is probably just one factor to take into account when choosing the number of implants to support a full arch prostheses. Of course in a patient with a history of high occlusal stress I would prefer to have more implants than less, as well as an old 40kg lady is probably a different load to implants than a young 100kg man. Unfortunately we still relay on personal preferences and opinions rather than evidence on this topic.... thanks again Maurice.

Carlo; excellent. What are your thoughts on the # of implants to support occlusal load in maxilla and mandible for a fixed restoration? This especially in light of the controversy with the All on Four concept?
Thanks again
Maurice

Sam Busich thanks! Yes if possible natural teeth propioception could greatly regulate forces transmitted to implants and prostheses

Carlo; Very nicely done. Glad that DentalXP was capable of introducing this topic through such an excellent presenter. Would you prefer to maintain natural canines when possible to "protect" off axis forces on implants??
thanks Sam

Implants, Overdentures, Hybrids? What’s Up With That?
It’s no secret that patients who have to wear dentures often have challenges. While it would be great to just put in 8 implants and four bridges in those toothless arches the financial realities of that extensive treatment prevail.

Comprehensive Management of Implant Anchorage in the Multidisciplinary Patient
In recent years, dental implants have become an accepted method of replacing missing teeth. Today millions of implants are placed annually to rehabilitate and reestablish patients’ occlusions. However, in many of these individuals, the teeth may be in less than ideal position to accept the integration of single implants or groups of implants with the remaining teeth. Many of these patients could benefit from orthodontics to reposition malposed teeth to enhance the overall occlusal scheme. However,…

Implant Prosthodontic Procedures for a Completely Edentulous Patient with a Cleft Palate
Cleft lip and palate maxillofacial prosthodontics is evolving and re-defining treatment approaches via the use of dental implants. A case report is presented demonstrating the use of the sinus lift procedure originally described by Tatum, the placement of osseointegrated root-form implants, and an implant-supported and -retained maxillofacial prosthesis.
