Near the final part of the video, Dr V said that he would prefer to graft collagen in the area where he was not going to place implant in order to reduce the bone volume. What form of collagen? Does he mean using collagen membrane or collaplug? If membrane, does he roll them up, then place bone graft next to them? Or just place several collaplugs?

Dear Dr Furnari
Its is not necessary to make little perforations, you just need to make sure you release the schneiderian membrane from the mesial wall of the sinus, wich is the main blood suply zone from the sinus. i always use bio-oss + PRGF, wich according to Dr Anitua's is the best option to fill up the sinus. i myself have excellent results with this technic. i only use piezo for implant osteotomy, when i'm crist splitting and pacing implants at same surgical time.

Att Dr. Salama: I took your course with Dr. Pikos in June, just two weeks ago I was able to use the Piezoelectric for a bilateral sinus lift. BTI lent me the PRGF system and the proceedure went well. I have the following questions? 1)Is it necessary to make little perforations on the inside of the sinus floor to increase the blood supply? I don't recall Dr. Stephen Wallace or any one else reccomending this. 2)Why does Dr. Vercellotti not like to use Bio-Oss in his Sinus lifts? I believe he said that he prefirs autogenous bone plus collagen only. 3) Lastly, do you like Piezo surgery for implant osteotmy ?

I agree with the advantages of the piezoelectric in sinus lift. But I have doubts and I would like to know your opinion, on the new tips for implant site preparation for piezoelectric ?

Platelet derived growth factors are now routinely utilized in reconstructive therapy. This presentation describes very detailed and evidence based guidelines for clinicians interested in enhancing their abilities in tissue engineering, especially as it relates to bone augmentation. Specifically, while successful bone augmentation requires the standard surgical parameters of space maintenance, low pressure on the grafts and tension-free flap closure, optimization of this goal requires management and enhancement of the local biological conditions with growth factors. Towards that end, platelet concentrates through the constant release of growth factors are able to promote and enhance new vascularization, provide plasma protein, normal lipidemia, as well as increased collagen and fibrin activity.

The Effect of Piezoelectric Use on Open Sinus Lift Perforation: A Retrospective Evaluation of 56 Consecutively Treated Cases From Private Practices
The lateral window approach to maxillary sinus augmentation is a well-accepted treatment option in implant dentistry. The most frequent complication reported with traditional techniques has been the perforation of the Schneiderian membrane, with erforation rates ranging from 11% to 56%. The purpose of this retrospective, consecutive case series from two private practices was to report on the rate of Schneiderian membrane perforations and arterial lacerations when a piezoelectric surgical unit was used in conjunction with hand instrumentation to perform lateral window sinus elevations.
