On the beautiful model, they could have just done a composite bridge between #8 and #10 instead of cutting the good teeth down. Not hard to do and it can be esthetic as well. Just basic bonding.

Interpositional Bone Grafts to Treat the Posterior Atrophic Mandible
This presentation analyzes the interpositional bone graft or inlay technique, discussing the reconstructive surgical approach. Horizontal osteotomy with the interposition of bone in the form of a “sandwich” involves raising a coronal osteotomized segment of the mandible, which is still attached to the lingual periosteum, and interpositioning a block bone graft. This technique guarantees a dual vascular supply to the inlay graft from the lingual periosteum and from the residual bone; it also allows optimum use of the native basal bone, which should be less prone to resorption. The advantages and disadvantages of the inlay technique are compared with other commonly used augmentation techniques in the management of posterior mandibular atrophies.

Soft tissue management is one of the key factors for successful implant restoration. Esthetics can be strongly influenced by the conditions of the peri-implant tissue. Substantial knowledge of tissue anatomy and biology is a fundamental prerequisite for surgical success. The present chapter introduces different techniques to improve the appearance of the soft tissue after implant placement. Cases of repositioned flaps, FGGs, and CTGs are presented along with biological considerations. It was our intention to present various indications and options to the clinician to achieve an increased amount of peri-implant KG or connective tissue thickness.
