rhBMP-2 Infuse and Titanium Mesh Utilization for Horizontal Augmentation in the Maxillary Lateral Incisior Region
Description:
In this video, Dr. Salama highlights a regenerative protocol utilizing rhBMP-2/ACS INFUSE and mineralized irradiated allograft bone, Miner-Oss in combination with a pre-fit titanium mesh apron and implant placement in a narrow ridge. Incision design and tension free closure are also featured.

Nelson: I can only speak of my experiences with BMP-2 and I have no adverse reactions that you are suggesting other than significant post-op edema. regards Dr. Salama

What do you think about Infuse adverse effects as bone ovegrowth, sterility, ectopic bone growth?...

Thank you Dr. Salama for you work. Do you have any follow-up images/video for this case?

Dear Dr Salama, you only fixed the titanium mesh from occlusal, will it be sufficient ?what if you fix it from buccal side also?will additive fixation reduce the risk of movement and exposue?tnx again

Kevin; Yes. The ACS collagen sponge is the carrier for the BMP-2 liquid. The titanium mesh can be purchased on XP from Neobiotech. They are CTi-Mesh that connect to the implant like an apron.
Yes, as there is a higher cost for the Infuse, I would charge more than just a normal particulate GBR procedure.
Good luck and thanks

so the collagen sponges is different than the infuse. the infuse is a liquid and the sponges are soaked in it? How much are these components the titanium mesh ? and the infuse? and you charge more than just a normal particulate graft right?

Leonid; A membrane is contraindicated with the use of BMP-2 as we wish to allow the passing of blood and nutrients to the bioactive material.
thanks Dr. Salama

Luigi and Jan; Great discussions. Yes, we must be concerned about higher risk of ti-mesh exposure but it is preferred when using BMP-2 for regeneration. I am not concerned about implant angulation as I confirmed placement with radiograph. Yes, we will thicken KT at 2nd stage uncovering and manage any Ti-mesh dehiscence if they occur. Stabilization of apical end of Ti-mesh is most likely not an issue after clot stabilization but I would prefer a Ti-mesh with apical "cleats" that can be tapped into cortical plate.
I am working with a company now to produce such a Ti-mesh.
Thank you both for a great series of real world Clinical issues.
Happy New Year
Dr. Salama

Peridontal Osseous Surgery and Root Resective Therapy
Historically, osseous surgery was performed to eliminate infected bone. Early in the 20th century clinicians realized that periodontally affected bone was not actually infected. Instead of an invasive bacterial infection, chronic inflammation was responsible for bone destruction. This finding changed the rationale for periodontal therapy and led to modern concepts of osseous surgery. Currently accepted approaches are based on guidelines, parameters, and definitions published by many highly respected…
