Root Canal Perforations - Diagnosis and Treatment - Part 2 of 3

Description:
In this dynamic video presentation, Dr. Leandro Pereira details the diagnosis and variable options for the management and treatment of root perforations. The advantages of CBCT in the diagnostic and evaluation phase as well as surgical microscopy in the treatment phase are discussed. A thorough algorithm that includes location, size and shape of the perforation is presented to guide the clinician in assessing the potential for success in clinical intervention. Materials as well as surgical vs. non-surgical treatments are also discussed.

Questions & Comments

Dear Dr. Pereira, thank you for a great presentation! I have a question concerning the placement of resin-modified glass-ionomer fillings (or any other filling materials for that matter) in order to restore slightly supra-crestal root canal perforations such as the one shown 7 minutes and 40 seconds into this video. Does this treatment option have the potential to violate the biologic width considerably enough to cause periodontal or aesthetic issues? Also, what is your material of choice in these cases?

Leandro Pereira56 - (8/14/2012 10:23 AM)

Dear Milind Saudagar, this patient is an Angle's Class I and has a very delicated mastigatory muscles. The prognosis is good. Have a look at the last slide of part 3 of this lecture. There is an algorithm explaining about prognosis. Hope you enjoy. If you have more questions, fell free to contact me.

fantastic video and case presentation sir. a small query, will this tooth be abel to withstand occlusal forces?

Leandro Pereira56 - (5/20/2012 7:05 PM)

No. In some cases there are a formation of a new periodontal ligament over the extruded MTA.

ASHOK GUPTA - (4/25/2012 9:45 PM)

can extrusion of MTA cause ankylosis

Three-Canal Maxillary Premolar Teeth: A Clinical Reality
To treat three-canal maxillary premolar teeth predictably, it is necessary to be aware of the clinical and radiographic anatomy. These teeth may also require special shaping and obturation techniques. The purpose of this presentation is to show the complete procedure of an endodontic treatment of a upper left second premolar with three root canals.

ProTaper Universal - Whatever You Thought, Think Again
In 1995, the opportunity was present to develop an improved set of NiTi files. The goal was to create a simple set of easy-to-use files that would significantly improve the benchmark for safety and efficiency. In collaboration with the engineers from Maillefer, Drs. Pierre Machtou, John West and I agreed to develop a set of files that would precisely duplicate the Schilder shaping concept.

Evolution and Integration of Current Restorative Endodontic Concepts

Evolution and Integration of Current Restorative Endodontic Concepts
The restoration of endodontically treated teeth was advocated by G.V. Black in 1869 followed by a one-unit post and crown by Richmond in 1878. Engineering principles indicate that structural strength on endodontically treated teeth is decreased by the removal of the roof of the pulp chamber, canal enlargement, and hence loss of integrity. The intrinsic strength of dentin can be diminished as a result of root canal treatment originated by decay, pathology, or trauma. When endodontic treatment is indicated,…

Endodontic-Restorative Symbioses: Diagnosis and Treatment
The success of endodontic treatment is comprised of proper diagnosis and treatment, which is easier to accomplish when the symptoms correlate to the clinical and radiographic findings. It is generally a more predictable treatment on anterior teeth as opposed to posterior teeth due to the number of canals and their internal configuration. A multidisciplinary approach has never been more essential to provide the patient with the best possible choices for treatment. Moreover, the general dentist and…
