In this dynamic video presentation, Dr. Leandro Pereira details the diagnosis and variable options for the management and treatment of root perforations. The advantages of CBCT in the diagnostic and evaluation phase as well as surgical microscopy in the treatment phase are discussed. A thorough algorithm that includes location, size and shape of the perforation is presented to guide the clinician in assessing the potential for success in clinical intervention. Materials as well as surgical vs. non-surgical treatments are also discussed.

Dear Dr. Pereira, thank you for a great presentation! I have a question concerning the placement of resin-modified glass-ionomer fillings (or any other filling materials for that matter) in order to restore slightly supra-crestal root canal perforations such as the one shown 7 minutes and 40 seconds into this video.
Does this treatment option have the potential to violate the biologic width considerably enough to cause periodontal or aesthetic issues? Also, what is your material of choice in these cases?

Dear Milind Saudagar, this patient is an Angle's Class I and has a very delicated mastigatory muscles. The prognosis is good. Have a look at the last slide of part 3 of this lecture. No. In some cases there are a formation of a new periodontal ligament over the extruded MTA.
