dear dr mazor,posterior maxilla is always a problem ,what is the reason for using tapered implants?

I have many cases done with minimal amount of residual bone height all done with simultaneous implant placement.As long as you use tapered implants and no denture-there should be no problem. In 1996 we published in the JOMI 9 year longitudinal study done with 731 sinuses and more than 2000 implants placed simultaneously regrdless of the alveolar bone height. As for the membrane it need not be fixated. As for splinting- I always connect the implants with the fixed prosthesis. The only differences in the surgical technique used today are:Using Piezo instead of the bur,Filling up the sinus before implant placement and the use of PRF together with the grafting material.

very delicate surgery.these type of cases are very common in india where bone qulity is very poor in posterior maxilla. do you suggest the collagen membrane has to be fixed, or it does not matter? when do you load these cases. what is your long term experience on these type of cases.? also what type of prosthesis do you suggest splint all implants together, or do single crowns on them?

Again, to follow the first question, why not allow the graft material to heal first, then place the fixtures 6-8 months later when better primary stability can be achieved?

After elevation of the sinus membrane there are two possibilities for placing the implants simultaneously with the grafting material. The first option is placing the fixtures half way, adding the grafting material in all aspects and then screw the fixtures all the way. The second option and that is how I do my sinus augmentations today is filling up the entire compartment and than screw the fixture all the way. Care should be taken to ensure adequate filling especially on the palatal medial aspect.

Tissue Engineering and Platelet Derived Growth Factors: Evidence Based Therapy
Platelet derived growth factors are now routinely utilized in reconstructive therapy. This presentation describes very detailed and evidence based guidelines for clinicians interested in enhancing their abilities in tissue engineering, especially as it relates to bone augmentation. Specifically, while successful bone augmentation requires the standard surgical parameters of space maintenance, low pressure on the grafts and tension-free flap closure, optimization of this goal requires management and enhancement of the local biological conditions with growth factors. Towards that end, platelet concentrates through the constant release of growth factors are able to promote and enhance new vascularization, provide plasma protein, normal lipidemia, as well as increased collagen and fibrin activity.

Computed tomography (CT) and the application of CT-based guided implant surgery allow clinicians to
provide enhanced precision and accuracy in implant surgery. Because of the difficulty in transferring a

Controlling Systematic Perioperative Anaerobic Contamination During Sinus-Lift Procedures by Using Metronidazole: An Innovative Approach
Analysis of tomodensitometric controls following sinus grafts clearly demonstrates a quite systematic lack of homogeneity. Sinus contamination by anaerobic bacteria seems almost unavoidable during bone graft surgery, and this problem may jeopardize the healing process.The aim of this study was to characterize in a systematic way the nonhomogeneities observed at 1,2,or 3 months post surgery within allogenous sinus grafts,and to assess the possible influence of a 0.5% sterile solution of metronidazole in corporated in the sinus bonegraft.
