Socket Preservation vs Root Submergence Technique

Description:
Socket preservation has been a common technique used in dentistry in order to maintain the volume ridge when the extraction is performed. New techniques and materials are used today in order to improve the outcomes like root submergence technique developed by Dr Maurice Salama and his team, where we submerge the root keeping intact the PDL and bundle bone responsible for maintaining the original bone architecture. Here both techniques are compared and described.

Thank you all for the nice comments and great questions. Dear colleagues, Dr Maurice Salama is the expert and first researcher on this technique and his approach is the most accurate and scientifically driven. While submerging the root ensures biological environment and no need for grafting it may develop a mild vertical defect, this is why is important to manage very well the root reduction. Thick Biotypes allow only 1-2mm of root reduction since a thick CT and epithelium is present, while thin biotypes may need more reduction due to lack of soft tissue thickness.

Excellent presentation! I was involved in discussions about necessity of submerged root covering. Although theoretically blood clot appeared as enough, I didn`t dare to relay only on blood clot. Thank you for sharing, very much enjoyed and appreciated!

The Socket-Shield Technique: First Histological, Clinical, and Volumetrical Observations after Separation of the Buccal Tooth Segment – A Pilot Study
The “socket-shield technique” has shown its potential in preserving buccal tissues. However, front teeth often have to be extracted due to vertical fractures in buccolingual direction. It has not yet been investigated if the socket-shield technique can only be used with intact roots or also works with a modified shield design referring to vertical fracture lines. The aim of this study was to assess histologically, clinically, and volumetrically the effect of separating the remaining buccal root segment in two pieces before immediate implant placement.
