"The Soft Block": Treating Severe Recessions Around Dental Implants - Part 1 of 2

Description:
Severe recessions around implants is a common issue. Unhappily this is a problem we have to face in our everyday practice. To deal with it we have to develop strategies to meet our patients expectations. Recessions and peri-implantitis come together very often. Being capable to improve the biological environment around a dental implant may have huge relevance in it´s long-term prognosis. Treatment algorithms proposed by some authors are often very complicated and sometimes frustrating in what comes to results. Filipe Lopes & Bernardo Mira Correa will try to present simple algorithms and biological ways for trying to solve this kind of clinical challenge, other than implant explantation that must be also, and so many times, considered as the first choice. This is intended to be the first of a series of posts on the subject. We will feature different soft tissue management strategies, including autologous grafts and alloderm. We will underline the importance of: diagnosis,PRGF ENDORET® graft, type of implant, grafting technique to use and allografts that can possibly help us on some challenging situations. Emphasis will be also held on tools, the BTI extraction kit, that allow us an easier removal of implants than ever before, reducing dramatically the biological cost of their loss and making it easier to place a new implant. At the end we will try to provide for a simpler treatment algorithm, that has been of good service for us, for some years now.

Hi Roy! I saw your question just now! Thx for watching. In fact this is a very special type of fully epithelized FGG (free gingival graft), so you don´t cover it. Until the end of october 2018 I will publish a brightbook on the technique.

Mansur Roy - (10/5/2017 5:07 PM)

Hi Flipe, Thanks for sharing the case. did you cover the CT graft entirely with the flap?

filipe lopes - (1/6/2017 12:01 PM)

Thx Mo. I posted 2 years and a half ago and the patient still smiles radiantly. That grey collar showing was really an issue for her. Up until now we were succesfull in avoiding expalnting the implant. See u soon in Vegas!

i am sorry but final position is not esthetic...

Maurice Salama - (7/24/2016 8:43 AM)

Excellent and just watched it for a 3rd time....thanks again Maurice

Hi Enrique Trevino! We invite you to also see: Soft Tissue Management - The Pink Contribution to Achieving Esthetic and Long Lasting Smiles, that we also published here on Dental XP! Its shows in detail, our protocol for grafting the donor site with PRGF! we hope you like it too! Best regards Bernardo

Enrique it's nice to see how Sonia Lezyi uses PRGF concomitant with extraordinary soft tissue grafting strategies.

Edison thanks for your kind words. Filipe.

Filipe, thank you for your kind and prompt response. This is what makes Dental XP so exciting and rewarding. You are one of the gifted presenters who are generous enough to share your knowledge and skills with the clinicians throughout the world.

Socket Shield; What Do We Know, What´s New?
The Socket Shield technique is a “new” procedure with 10 years of documented successful cases worldwide. Lately we have done some new proposals like the way we extract the lingual slice, the relation of the shield level to the bone, the use of miniflaps, the distance between implant and shield just to mention some. On the other hand, the only minor “problem” with this technique is the “external shield exposure” and here I propose the method of successful treatment. We will see the convenience of applying SS to molars also on the light of the paper of Dr. Mataraso from 2009. The molar SS is a viable alternative rather than ridge preservation techniques.

Diagnosis, Planning, & Treatment of Altered Passive Eruption; From a Periodontists Perspective
This presentation will teach how to diagnose cases of altered passive eruption based on the radiographic evaluation (with periapical rx and CBCT) and how to plan and document a surgical procedure aimed to ‘uncover’ the beautiful and natural hidden enamel. In a world where esthetics is becoming more and more a concern a simple periodontal plastic surgical procedure can re-establish esthetic and balance to the patients affected by this condition.

Microsurgical Soft Tissue Reconstruction for Teeth & Implants - Part 2 of 2
Esthetics plays a key role in a modern society and the average dental health of the population is improving every day. In such environment, the demands of dental treatments are different and dentists should approach their patients in a different way: they should be prepared to implement different and modern techniques around teeth and implants. Soft tissue grafting techniques provide the dentist with an armamentarium that makes possible to prevent and solve many problems and complications around teeth and implants, making more predictable the treatments results. This presentation is focused in the importance of soft tissue grafting techniques around teeth and implants and how to do it in a predictable, modern and a minimally invasive way. We present many different techniques illustrated with clinical cases.

Peri-Implantitis: Diagnosis, Etiology and Treatment
The dental implant therapy has evolved both in implant surface and design. The reason for the initial crestal bone loss has been proven and the risks of having a peri-implantitis have increased due to the early exposure of roughened surface. The prevalence of peri-implantitis is at a range of 28 to 56%. The role of dental professionals nowadays is to know how to interpret the signs of inflammation and establish the diagnosis of peri-implantitis, which is the most challenging, and be able to treat it properly since this is becoming a more needed therapy.

The Socket Shield Technique - Have the Rules of the Game Changed in Aesthetic Zone Implant Therapy? Premium Member Content

The Socket Shield Technique - Have the Rules of the Game Changed in Aesthetic Zone Implant Therapy?
Implant therapy is in the age of being increasingly aesthetically driven, not merely restorative. With the increasing popularity of immediate implants, particularly with anterior tooth extraction, the relevance of socket changes following extraction has come to the fore. Contrary to what was originally believed, the installation of an immediate implant does not arrest bone loss, and immediate implants in the aesthetic zone are prone to recession of bone and soft tissue as a result of bundle bone resorption which is obligatory with tooth extraction.
