"The Soft Block": Treating Severe Recessions Around Dental Implants

Description:
Implants have been one of the major breakthroughs in dentistry. Tissue collapse around dental implants is our number one enemy as it gives rise to serious problems that go from inflammation, through many times chronic infection, all the way down to severe recession, seriously compromising not only esthetics, but also function and our patients well being. Strategies have been developed to help clinicians to try to avoid collapse. Among the most important are procedures such as: grafting the gap; GBR with several biomaterials and barriers; flapless approaches; immediate implant placement after tooth removal with provisionalization using CAD-CAM & 3D technologies and ultimately PET strategies also combined or not with immediate load. Unfortunately severe recession around dental implants are still a problem clinicians have to face in their everyday practice. Recessions and peri-implantitis come together very often. To deal with it we have to develop strategies to meet our patients expectations. Being capable to improve the biological environment around a dental implant may have huge relevance in it´s long-term prognosis. Treatment algorithms proposed by some authors are often very complicated and so many times frustrating in what comes to results. A biological approach will be presented to try to solve this kind of clinical challenge, other than implant explantation that must be also, and so many times, considered as the first choice.

this is just a free gingival graft covering rough implant surface?? Did i miss anything?? Can we roughen the implant with bur??

This is a great presentation! If you would be please be so kind I have the following questions: 1. 0.2μRa has been shown to be the threshold value regarding composite roughness with regards to plaque adhesion,on the other hand a well polished surface has shown variable results with regards to CT adhesion (Chu S et al. dual zone article, where some cases showed bleeding and some no upon temporary prosthesis removal). Thus the tissue you show on your last case with the custom healing abutment, is this CT adhesion effect or an irritated/possibly inflamed tissue? My guess is that only histologic evaluation of cells present would give a definitive answer but I am interested in your opinion 2. With regards to Laser lock design, it is my understanding that the effect is due to the different micro geometry and not to the roughness of the surface. Surface treatment modalities have an effect on the roughness and is irrelevant to the presence of the groves, am I wrong? I believe that you have raised an important parameter which is the roughness threshold of biocompatible materials with regard to CT response. Very interesting! Thank you Ioannis

A Review of Clinical Applications for Soft Tissue Allografts & Autografts: Solutions for Teeth, Implants, and Dentures
The presentation focuses on the clinical applications for acellular dermal graft (ADG) materials. ADGs have been commonly presented and discussed to provide solutions for treating gingival recession. Compared to autogenous grafts, they have the advantage of minimizing pain, discomfort, and surgery time for patients by avoiding an intraoral donor site such as the palate. In addition to treating gingival recession, ADGs also have uses in bone grafting surgeries such as socket grafting and ridge augmentations (GBR) when they are utilized as a barrier membrane. Finally, ADGs can be combined with bone grafting materials to aid in the augmentation of edentulous ridges to aid denture support. While this last application is less common, it's use is worthy of discussion.

Failure of a natural tooth may not permit placement of an implant at the time of extraction due to insufficiency in available bone to house the implant. Reconstruction of the extraction socket frequently involves both hard and soft tissue augmentation to provide a site that can house the implant and ridge contours that mimic the adjacent natural anatomy. This situation becomes more problematic in the maxillary anterior due to the anatomy and the lower density of the bone of the premaxilla. The solution is the interpositional vascularized augmentation neogenesis (IVAN), which consists of hard tissue grafts, various barrier membranes, and closure with the pediculated connective tissue graft (PCTG). The modified IVAN (mIVAN) technique achieves the necessary goals and may be used in both delayed and immediate placement situations.

Maintaining Interimplant Crestal Bone Height Via a Combined Platform-Switched, Laser-Lok Implant/Abutment System: A Proof-of-Principle Canine Study
Interimplant papillae are critical for achieving esthetic implant-supported restorations in the maxillary esthetic zone. Stable papillary anatomy, however, depends upon a stable volume underlying crestal bone for support. Multiple studies have documented a critical interimplant distance of 3mm under which crestal bone resorption occurs. This preclinical proof-of-principle canine study examines a novel implant-abutment system design, combining platform switching with precisely configured laser-ablated abutment and implant microgrooves to maintain interimplant crestal bone at interimplant distances of 2 and 4 mm. Results of this initial preclinical study suggest that it is possible through precise implant/abutment design modifications to place adjacent implants at distance of 2 to 4 mm without inducing subpapillary crestal bone loss.
