Tissue Engineering and Platelet Derived Growth Factors: Evidence Based Therapy

Description:
Platelet derived growth factors are now routinely utilized in reconstructive therapy. This presentation describes very detailed and evidence based guidelines for clinicians interested in enhancing their abilities in tissue engineering, especially as it relates to bone augmentation. Specifically, while successful bone augmentation requires the standard surgical parameters of space maintenance, low pressure on the grafts and tension-free flap closure, optimization of this goal requires management and enhancement of the local biological conditions with growth factors. Towards that end, platelet concentrates through the constant release of growth factors are able to promote and enhance new vascularization, provide plasma protein, normal lipidemia, as well as increased collagen and fibrin activity.

Dr.Choukroun, Have you ever thought about preparing PRF from bone marrow aspiration. there has been some resarch on the advantages of PRP prepared from bone marrow aspiration. i have employed your technique in cleft surgery in alveolar cleft grafting and would love to have an extended discussion with you regarding the results. it is a pleasure to see and hear you so thank you dental xp.

Deer Dr. Choukroun,Do you suggest Shock wave for a routine procedure before implant surgery?and what is the protocol?

Joseph Choukroun - (8/6/2012 11:39 AM)

Namita, 1. You can draw the blood at any time: before or during the surgery. The essential is to remove the clots after the spin and put them in the PRF Box. 2. The local anesthesia and the medications do not interfer on the PRF clot quality.

Namita Dutta - (8/5/2012 12:23 AM)

Dear Dr Choukroun, Recently I purchased LPRF kit from Dental XP. I want to know if the blood can be drawn during the surgery for example after placing implants and prior to grafting as I will have phlebotomist draw the blood initially till my staff gets trained and she can come later due to her schedule for one of my patients. Does local anesthesia pre-med with antibiotics glucocorticoids and oral conscious sedation (trizolam)affect the quality of fibrin? Appreciate your response. Dr. Namita Dutta

Tnx a lot Dr Choukroun, the point ou mentioned about LDL chlosterol wa very important to notice.and the fct that the pressure will reduce angiogenesis.i learned a lot.tnx again.

Alireza, I prefer to use mineralized. In GBR, it makes sense to use cortico-cancellous. In the sinus, as the situation is easier to make bone, you can use anyone. you need only to maintain the schneiderian in upper position.

Alireza Firouzeh - (6/20/2012 5:02 PM)

Dear DR CHOUKROUN
First of all I appreciate your great performance in lecture that was so helpful for me in learning new things but my question is in regard to what kind of allograft do you prefer to utilize in combination with PRF??? I mean that mineralized allograft or demineralized type or even partially demineralized allograft is your priority and why??Is there any differences in applying them depends on the GBR area???For example,in sinus graft do not you think that mineralized allograft has better consistency and biomechanical properties compare to demineralized type???
I would be glad hearing from you ASAP.
thanks a lot for your response

Managing the Extraction Site - Optimal Preservation and Enhancement Techniques
There is rarely a day that clinicians do not have to deal with an extraction site. If implant therapy is a consideration, then bone and soft tissue quantity and quality must be factored into treatment options. The ideal opportunity for bone preservation exists at the time of tooth extraction. All too often this opportunity is lost. Extraction techniques will be demonstrated that not only preserve but augment the amount of alveolar bone. The importance of piezosurgery, atraumatic surgical technique, the use of bone grafts, resorbable membranes and growth factors will be elucidated along with soft tissue enhancement.
