Vertical Bone Augmentation Using a Tunnel Approach Step by Step - Part 2 of 2

Description:
Bone atrophies are one of the most challenging situations in modern implantology. Several techniques have been proposed to be able to place implants in those atrophic situations. This first presentation will show step by step the Split Bone block technique as described by Dr. Khoury to perform horizontal bone augmentation procedures. Bone biology is also explain in order to understand why this technique offer us some great advantages. The second presentation will focus of vertical bone augmentation procedures using this same technique but through a tunnel approach. The advantages of this approach will also be described in this presentation.

Hi Miguel! No, no membranes are used. We always keep in mind the concept of Revascularizable membranes” We place no obstacles between the periostium and the thin bone block. Neither horizontal or vertical. In vertical augmentation if we have at least 4-5 mm of KT we perform a Kazanjians Vestibuloplasty to achieve fond of vestibule and attached mucosa (Not KT) if the band of KT is less that 3-4 mm we perform a FGG no be able to have some quality tissue around implants. Hi Achille! No experience with Xenoblocks, sorry! Could they be used? I really think so, but we have to bear in mind the concept “biological revascularizable membranes”. will they act as so? That would be an interesting topic!

Beautiful work. 2 questions if I may: -Is a free or CTG more than likely always recommended as a result of this technique. -please explain reentry soft tissue incision. Is it preparing future bed for mentioned graft procedure? Would you cover the occlusal aspect of a horizontal graft if no occlusal block used/needed? That was three my bad Congrats again!!

Hi David! Piezo devices can also be used for harvesting the bone block without any doubt. Microsaw is faster, but if you feel more comfortable with piezo go for it! Be sure you have some very thin inserts to split the bone block. Thanks for watching the presentation. Jose Mompell

Replacing both missing maxillary interior teeth is particularly challenging, especially in compromised sockets. The case report describes the management of an 18-year-old female patient, who suffered avulsion of both maxillary central incisors at 7 years of age. The multidisciplinary implant technique, called Immediate Dentoalveolar Restoration (IDR), included extraction of the injured teeth and a single procedure for immediate implant placement and restoration of the compromised sockets after root fracture and peri-apical lesion development were detected during orthodontic treatment. Successful esthetic and functional outcomes and reestablishment of the alveolar process after bone reconstruction were observed during the 3-year follow-up period. The predictable esthetic outcomes and soft and hard tissue stability that can be achieved following IDR are demonstrated.
