Soft Tissue minimally invasive procedures can often be utilized to reconstruct large defects in the anterior region rather than rebuild the site with Bone which would entail multiple procedures over several months and increase the morbidity for the patients as well as risk for the clinician. This presentation highlights numerous soft tissue procedures that can augment large deficiencies in the esthetic zone as a preference over bone augmentation. Roll, pedicle grafts, and tuberosity tissues can be predictably utilized to maximize our results and limit time and risk for our patients. Tooth replacement, implant failure restitution and immediate implant placement SVG grafting will be reviewed in detail.

This clinical case aimed to achieve two main objectives. The first was to determine if the use of only calcium phosphosilicate bone graft as a regeneration material (with no autologous bone added) on a severe vertical and horizontal mandibular defect would allow enough bone to be obtained to enable the placement of dental implants. The second objective was to determine histologic characteristics of the regenerated site after a healing period of 10 months.

Immediate implant placement is a well-recognised and successful treatment option after tooth extraction.1,2 Success rates for both immediate and delayed implant techniques are comparable; however, the literature reports that one can expect buccal collapse of the original buccal volume and midfacial recession of at least 1mm after immediate implant placement, possibly worse in thin gingival biotypes.3

Clinical and Esthetic Outcomes of the Surgical Veneer Grafting Protocol in the Anterior Maxilla
A multicenter cohort clinical investigation evaluated the change in horizontal ridge dimension associated with immediate placement and provisionalization in anterior extraction sockets. Following the surgical veneer grafting (SVG) protocol, a combination of a bone and connective tissue graft (CTG) was performed in 37 patients to compensate for the natural hard and soft tissue remodeling to achieve a sustainable and predictable esthetic outcome in patients with a thing gingival phenotype. The patients were followed for an average of 39 months (range of 10 to 58 months). At the four-year recall, 61% of the fixtures were evaluated. All 37 implants were successfully integrated, demonstrating stable and healthy peri-implant soft tissues as documented by standard clinical parameters. The results showed an average volume gain of 0.68 mm in the tissue zone, compared to the initial situation.

The Critical Diagnostic & Tx Planning Aspects of Full Arch Implant Therapy
Full Arch Implant Dentistry has become a significant aspect of the modern dental practice. Our ability to replace a failing or missing dentition immediately with dental implants and a functional restoration has improved the quality of life and overall health of our patients. These treatments have become increasingly popular as they reduce chairtime and maximize the cosmetic aspects. With the abundance of cases, it is imperative that we avoid mistreatment or overtreatment. This presentation describes a step by step diagnostic agenda to properly evaluate each patient utilizing a full digital workflow including, CBCT, Intra-oral scans, full mouth radiographs and photos as well as clinical evaluation of soft tissues, bone and positional occlusion. The process of identifying and selecting the proper cases and designs for each individual situation will be thoroughly reviewed as well as to understand the limitations and possible complications that may arise.

Diagnosis and treatment plan properly these cases play a pivotal role for the final successful outcome. In fact, CBCT software analysis together with all the prosthetic evaluation in needed prior to the surgical day will be highlighted, bringing then the focus of the presentation specifically on the diagnostic, surgical, and prosthetic procedures that can improve the predictability of the overall procedure. Which implant geometries are to be used? Which is the rationale behind the choice of placing straight or tilted implants? How many implants are needed and where? Abutment selection? Space requirements? Material selection? Bone quality issues and how to best incorporate new digital software and planning tools.

Buccal Sliding Palatal Pedicle Flap Technique for Wound Closure After Ridge Augmentation
One standard approach for wound closure after ridge augmentation is coronal flap advancement. Coronal flap advancement results in displacement of the mucogingival junction and reduction of the vestibulum. In the maxilla, a buccal sliding palatal flap can be applied for primary wound closure after ridge augmentation. The dissected part of the palatal connective tissue is left exposed, thus eliminating or reducing the amount of the coronal flap advancement respectively and increasing the amount of keratinized gingiva. In combination with guided soft tissue augmentation, this flap design enables a three-dimensional peri-implant soft tissue augmentation.

A Decade of the Socket-Shield Technique: A Step-by-Step Partial Extraction Therapy Protocol
Ten years have passed since Hürzeler and coworkers first introduced the socket-shield technique. Much has developed and evolved with regard to partial extraction therapy, a collective concept of utilizing the patient’s own tooth root to preserve the periodontium and peri-implant tissue. The specifications, steps, instrumentation, and procedures discussed in this article are the result of extensive experience in refining the socket-shield technique as we know it today. A repeatable, predictable protocol is requisite to providing tooth replacement in esthetic dentistry. Moreover, a standardized protocol provides a better framework for clinicians to report data relating to the technique with procedural consistency. This article aims to illustrate a reproducible, step-by-step protocol for the socket- shield technique at immediate implant placement and provisionalization for single-rooted teeth.

Managing Esthetic Implant Complications
Dental implant success today is judged not only by osseointegration but also by bone, tissue stability and of course long term esthetic results. Cosmetic predictability can often be difficult to attain, and esthetic implant failures can be multifactorial and patient management issues. Once esthetic implant failures occur, many cannot be fully corrected. Some complications must be addressed by an interdisciplinary dental team. In this summary of case reports, surgical considerations are provided, including cases of facial asymmetry/recession due to facial implant placement or bone loss resulting from technique/treatment failures, as well as papillary deficiencies. Restorative considerations for correcting failures are also discussed.
