Summary:

Endosseous dental implant surgery in the posterior maxilla requires the consideration of multiple factors, especially those of anatomic origin. Due to the reduced bone quality of the posterior maxilla, edentulism in this sextant often results in a resorbed osseous structure and a pneumatized maxillary sinus. When <5 mm of residual bone remains between the alveolar crest and the maxillary sinus, the traditional treatment option of choice prior to implant placement is subantral augmentation, more commonly known as the lateral window sinus lift. This approach to increasing bone available for placing implants uses a ‘‘window’’ in the lateral bony wall of the maxillary sinus to gain access to the underlying Schneiderian membrane. Although implant survival rates associated with this procedure routinely exceed 90%,the lateral window sinus lift remains a technique-sensitive procedure due to the high risk of Schneiderian membrane perforation and hemorrhagic complications, the latter of which is associated with the inadvertent laceration of the intraosseous arterial supply to this region.

Evidence suggests that smoking is detrimental to the survival of dental implants placed in grafted maxillary sinuses. Studies have shown that improving bone quantity and quality, using rough- surfaced implants, and practicing good oral hygiene may improve outcomes. In this prospective study, the long-term survival rates of implants placed simultaneously with sinus grafting in smokers and non- smokers were compared.

Minimally Invasive Transcrestal Sinus Augmentation(MITSA)
In this video Dr Kher discusses a method of transitioning from the lateral window sinus elevation procedure to a less invasive transcrestal sinus elevation in cases with alveolar ridge height between 3-5mm. The technique is based on using an ossteotome mediated lift combined with a novel delivery system of injecting the calcium phosphosilicate putty graft material. Dr Kher presents a case series wherein lifts in excess of 10mm were consistently achieved using this technique. A study comparing the increase in alveolar ridge height with the novel transalveolar technique and the conventional lateral window lift is also a part of the presentation.

The lateral window osteotomy (LWO) has been the most frequently utilized and published approach to sinus augmentation, presumably because there are no bone height restrictions and it provides the surgeon with more visual control; it is however, considered to be invasive, time consuming ,expensive and associated with patient morbidity.
