Immediate Loading of Bilaterally Splinted Titanium Root-Form Implants in Fixed Prosthodontics

Summary:
A load-free healing period has been advocated as a prerequisite to achieving osseointegration. This article reports two cases in which immediate loading of a specially designated additional or "expendable" set of titanium root-form implants was successfully utilized to support provisional fixed restorations in the maxilla and the mandible. This immediate-loading protocol is suggested as a reliable adjunctive therapeutic modality for offering implant patients access to fixed interim restorations during the healing phase of the primary fixtures. Another advantage of this approach is that it provides protection from potential transmucosal overload of the primary implants as well as any sites undergoing osseous regenerative procedures (Int J Periodont Rest Dent 1995; 15:345-361.)

When immediate implant placement is considered
for teeth with close proximity to the sinus floor, apical
extension of the osteotomy is significantly limited, and often
a staged approach is used. Implant placement into fresh extraction sockets and sinus floor manipulation using bone-added osteotome sinus floor elevation with implant placement
are techniques most often used independently or sequentially.
In this care report, immediate implant placement with simultaneous osteotome sinus floor elevation is an advantageous
combination of two successfully used techniques. This combined
approach can significantly reduce the treatment time
for implant therapy in teeth with close sinus proximity and provide the operator with the ability to place implants of desired length.

Sinus Lift & Immediate Implant Placement: "The Easy Protocol"
Sinus lift is still a common procedure in 2017. Many alternative techniques to sinus lift have been published to avoid invading this anatomical space, such as tilted implants with angled prosthetic abutments, extra short implants, distal pontic cantilever, zygomatic or tuberosity implant placements. Partial Extraction Therapies: From Failure to Everyday Practice - Part 2 of 4
Tooth extraction is usually followed by the resorption of support tissues that may compromise the aesthetic and functional prognosis of the final rehabilitation. There have been numerous publications suggesting Partial Extraction Therapies contribute to the maintenance of the alveolar ridge dimensions. Peri-Implantitis: Diagnosis, Etiology and Treatment
The dental implant therapy has evolved both in implant surface and design. The reason for the initial crestal bone loss has been proven and the risks of having a peri-implantitis have increased due to the early exposure of roughened surface. The prevalence of peri-implantitis is at a range of 28 to 56%.
The role of dental professionals nowadays is to know how to interpret the signs of inflammation and establish the diagnosis of peri-implantitis, which is the most challenging, and be able to treat it properly since this is becoming a more needed therapy.
