Maintenance of Alveolar Ridge Dimensions Utilizing an Extracted Tooth Dentin Particulate Autograft and Platelet-Rich Fibrin: A Retrospective Radiographic Cone-Beam Computed Tomography Study Maintenance of Alveolar Ridge Dimensions Utilizing an Extracted Tooth Dentin Particulate Autograft and Platelet-Rich Fibrin: A Retrospective Radiographic Cone-Beam Computed Tomography Study

Summary:

This study utilized radiographic comparative analysis in order to evaluate dimensional ridge changes four months after tooth extraction and immediate grafting with mineralized dentin particulate autograft and chopped platelet-rich fibrin. Fifty-eight extraction sockets with up to 2mm of missing buccal bone in the coronal aspect compared to the lingual bone were included. Graft material was covered with either a platelet-rich fibrin membrane or collagen sponge with no effort to achieve primary closure. The dimensional changes of the ridge were assessed on cone-beam computed tomography (CBCT) images acquired prior to extraction and four months later. The reduction in the buccal bone plate thickness 1 mm, 3 mm, and 5 mm below the buccal crest was −0.87 ± 0.84 mm, −0.60 ± 0.70 mm, and −0.41 ± 0.55 mm, respectively. The mean ridge width changes 1 mm, 3 mm, and 5 mm below the crest were −1.38 ± 1.24 mm, −0.82 ± 1.13 mm, and −0.43 ± 0.89 mm, respectively. The average mid-buccal bone height gain was +1.1%, while the mid-lingual height gain was 5.6%. A mineralized dentin autograft with platelet-rich fibrin is effective in preserving post-extraction alveolar ridge dimensions.

Use of Bone Grafts for the Enhancement of a GTR-Based Root Coverage Procedure: A Pilot Case Study

Use of Bone Grafts for the Enhancement of a GTR-Based Root Coverage Procedure: A Pilot Case Study
The use of guided tissue regeneration (GTR) procedures for the treatment of gingival recession has shown encouraging results and is gaining clinical acceptance. However, attaining space maintenance beneath the membrane remains a problem for clinicians. Hence, the purpose of this pilot case study was to evaluate the effect of adjunctive demineralized freeze-dried bone allograft (DFDBA) placement during collagen membrane GTR-based root coverage procedures. Five patients with Miller Class I or II defects…

CBCT as an Interdisciplinary Diagnostic and Treatment Planning Tool
The era of in-office Cone Beam Computed Tomography (CBCT) has unequivocally transformed interdisciplinary dentofacial therapy (IDT). While in-office CBCT imaging may be the latest technological breakthrough enhancing opportunities for treatment planning, it is diagnosis that has remained the constant variable critical for predictable patient outcomes.

Biologic Strategies to Enhance Clinical and Aesthetic Success in Oral Implantology
This webinar will discuss clinical strategies for treatment of the complex implant site involving bone grafting and implant placement. Our understanding of cellular pathways, and our ability to control the wound response, expands our capacity to effect better aesthetic outcomes and compress treatment time. The reduction of the inflammatory phase of tissue regeneration speeds up osseointegration, prevents crestal bone loss, maintains papillary form, increases the density of peri-implant bone and increases tissue biotype. This enhanced tissue response allows us to predictably treat even the most complex surgical cases, compress time to final reconstruction, and to maintain these favorable tissue outcomes over an extended period of time. The use of dental implants with advanced biologic features and the incorporation autologous growth factors will be demonstrated.

The Utilization of Advanced Flap Management & GBR to Large Alveolar Ridge Defects
Proper flap management is critical and key to obtain tension-free flap closure at large 3D ridge augmentation sites. The objective of this presentation is to highlight the most relevant clinical aspects to reduce tension during wound closure, therefore reducing associated complications, namely flap dehiscences that predispose our patients to post operative infection and reduced bone formation. Several clinical tips will be highlighted via multimedia and video to help the clinician increase their own predictability with these kind of procedures. Clinical cases will be presented to demonstrate the steps discussed in detail.
