Summary:

Suture selection is based on a knowledge of the physi cal and biologic characteristics of the suture thread in relationship to the healing process. In addition, accurate apposition of the flap is significant to patient comfort, hemostasis, reduction of the wound to be repaired, and prevention of unnecessary bone destruction. If flaps are not approximated and inadequate hemostasis is present, blood and serum may accumulate under the flap, delaying the healing process by separating the flap from the underlying bone.

The Management of Recession Midfacial to Immediately Placed Implants in the Aesthetic Zone
Immediate placement and loading protocols are the most technique sensitive and at the highest risk of complication, especially in the aesthetic zone. The peri-implant tissues undergo a resorption pattern that may see exposure of parts supporting the restoration, otherwise intended to be submerged, with extreme aesthetic compromise or even complete failure of treatment. The literature is not definitive in terms of any one treatment modality to recover such a complication. Grafting the exposure by a guided bone regeneration technique and an adjunct soft tissue augmentation could well restore an unaesthetic recession exposure. Recovery of midfacial recession by soft tissue augmentation alone may be successful, as is presented hereafter.

Diameter Selection of Implants Placed in Extraction Sockets: A New Approach
Implant placement in extraction sockets is considered a daunting challenge due to offering risks of bone remodeling and consequent gingival alterations. Objective: This study aims at proposing a protocol for selecting the diameter of upper anterior osseointegrated implants placed in extraction sockets. This protocol was based on the bucco-palatal dimension of the socket and allows a 3-mm gap to form between the implant and the outer surface of the buccal bone wall. Such gap must be filled with biomaterial or autograft so as to increase the predictability of long-term results.

The computer guided implant surgery and the x-guide navigation systems introduced a novel minimally invasive concept in the treatment of total edentulous and terminal dentition patients, with new perspectives based on bone graft-less rehabilitation and low morbidity implant surgery. Moreover with diagnostic data obtained from CBCT and extra and intra-oral optical surface scanning, a novel digital pathway based on facially driven virtual diagnostic waxing, prosthetically driven surgical plan, digitally printed surgical template, and prefabricated implant-supported screw-retained interim restorations can be realized in a predictable manner. The challenging interplay with the surrounding gingival framework, often overlooked in the extensive rehabilitation supported by tilted implants, will be highlighted through a step by step clinical protocol to deliver a natural gingival architecture and a direct pink free prosthetic emergence.

Current Concepts for the Preservation of the Post-Extraction Alveolar Ridge
After tooth extraction, bone resorption of varying amounts always occurs as the edentulous site of the alveolar process undergoes both qualitative and quantitative changes. Different techniques have been presented in literature in order to preserve the post-extraction site, including the use of barrier membranes, tri-dimensional matrix and bone fillers. An “open-healing” approach is nowadays commonly used, combining a proper management of soft tissues with biological integration of biomaterials. Once hard tissue has been successfully augmented, an adequate soft tissue thickness becomes mandatory in order to create healthy and stable peri-implant tissues. An innovative volume-stable collagen matrix provides the opportunity to treat soft tissue defects around implants as an alternative to connective tissue graft.
