The impacted upper central incisor can be classified depending on: mesio-distal position, occluso-gingival position, midline angulation, degree of root formation. If early intervention is not enough. There is the need for surgical exposure and orthodontic traction. Among possible treatment complications are: esthetic gingival sequelae, ankylosis and root resorption

Buccal Sliding Palatal Pedicle Flap Technique for Wound Closure After Ridge Augmentation
One standard approach for wound closure after ridge augmentation is coronal flap advancement. Coronal flap advancement results in displacement of the mucogingival junction and reduction of the vestibulum. In the maxilla, a buccal sliding palatal flap can be applied for primary wound closure after ridge augmentation. The dissected part of the palatal connective tissue is left exposed thus eliminating or reducing the amount of the coronal flap advancement respectively and increasing amount of keratinized gingiva. Partial Extraction Therapies: From Failure to Everyday Practice - Part 2 of 4
Tooth extraction is usually followed by the resorption of support tissues that may compromise the aesthetic and functional prognosis of the final rehabilitation. There have been numerous publications suggesting Partial Extraction Therapies contribute to the maintenance of the alveolar ridge dimensions. These techniques consist in intentionally preserving a buccal root fragment in order to avoid tearing the periodontal ligament and loosing the bundle bone, which leads to bone resorption. It has been reported that these are very sensitive techniques and may lead to some complications. In order to avoid these complications, the dental surgeon must know which cases can be treated with Partial Extraction Therapies and how to correctly perform them. In these presentations we will show what are the indications and contraindications, how to perform these techniques, the literature that sustains it, how can we benefit from digital technology to today's world, etc.

In this clinical case I had to restore the upper right jaw where the tooth number 14 and 15 had to be extracted. The 16 and 17 were inserted 6 months ago with crestal sinus lift using the Magnetic Mallet as well. Same on the other side of the maxilla. Crown removing by the magnetic device, palatal tooth portion extracted by the magnetic mallet and implant inserted by using the magnetic mallet for the osteotomes preparations and the implant insertions (15) to achieve the adequate primary stability for the immediate loading.

3D-Bone Regeneration in the Esthetic Zone: Fundamentals, Indications & Surgical Techniques Step-by-Step
The implant treatment on the esthetic zone aims healthy, functional and also esthetic results. In order to obtain an esthetic result is mandatory that the implants are placed in optimal 3D position and completely surrounded by healthy bone. Due to the alveolar ridge destruction caused by trauma and dental or implant infections, often bone availability is not enough to obtain a good esthetic result. In order to treat predictably such cases there are surgical techniques that allow a complete 3D reconstruction of the alveolar ridge. Of all those techniques described to regenerate 3D bone defects (with vertical and horizontal components) there are two that are more supported by scientific literature: Guided Bone Regeneration using PTFE non-resorbable membranes and ultra-fine cortical bone blocks (Khoury´s technique).

Socket Shield Preparation: An Oral Surgeon's Perspective
Periodontists and general practisioners are used on working with high speed, whereas oral surgeons are used to preparing hard tissue utilizing a straight handpiece. One can object that a straight handpiece is causing more vibrations and may dyslodge the shield. However, in our experience (myself:>300 socket shield procedures, Richard Martin, Michael Pikos) it works very well. This is not recommendation for the periodontists and GP to start to prepare socket shield utilizing a straight handpiece, but the explanatory video for oral and maxillo-facial surgeons used on a straight handpiece.

It’s impossible to heal well and fast when the tissue is in oxidative stress.. It occurs when there is an excess of oxidants or deficiency of antioxidants: smokers and diabetics have issues because they are in chronic oxidative stress... Tension free flap closure in mandatory. Otherwise, soft tissue and bone will fall in oxidative stress. The solution is to suture the flaps with an adequate technique. In this presentation, we show how to achieve soft tissue immobility and fixed buccal flap. How to manage the sutures? When to remove them? What’s the best material? How to handle a flat knot and the following knots? How to increase the soft tissue thickness? etc… All details and clinical follow-up are described in this short presentation.

Successful esthetic zone treatment must begin with a well thought-out space management plan. This plan involves a three dimensional adjustment of the periodontal attachment, with a possibility of root submergence and partial extraction technique. Following this, a decision is made concerning the timing and position of implant placement, and an adequate hard tissue foundation can be restored using GBR with titanium mesh and collagen membrane.
