The Socket Shield Technique - Have the Rules of the Game Changed in Aesthetic Zone Implant Therapy?
Implant therapy is in the age of being increasingly aesthetically driven, not merely restorative. With the increasing popularity of immediate implants, particularly with anterior tooth extraction, the relevance of socket changes following extraction has come to the fore. Contrary to what was originally believed, the installation of an immediate implant does not arrest bone loss, and immediate implants in the aesthetic zone are prone to recession of bone and soft tissue as a result of bundle bone resorption which is obligatory with tooth extraction.

Upper central incisor impaction has functional, esthetic and psychologic implications. Causes of the impaction can be divided in obstructive and traumatic. Tooth extraction is usually followed by the resorption of support tissues that may compromise the aesthetic and functional prognosis of the final rehabilitation. There have been numerous publications suggesting Partial Extraction Therapies contribute to the maintenance of the alveolar ridge dimensions. These techniques consist in intentionally preserving a buccal root fragment in order to avoid tearing the periodontal ligament and loosing the bundle bone, which leads to bone resorption. Buccal Sliding Palatal Pedicle Flap Technique for Wound Closure After Ridge Augmentation
One standard approach for wound closure after ridge augmentation is coronal flap advancement. Coronal flap advancement results in displacement of the mucogingival junction and reduction of the vestibulum. In the maxilla, a buccal sliding palatal flap can be applied for primary wound closure after ridge augmentation. The dissected part of the palatal connective tissue is left exposed thus eliminating or reducing the amount of the coronal flap advancement respectively and increasing amount of keratinized gingiva. In combination with guided soft tissue augmentation, this flap design enables a three-dimensional peri-implant soft tissue augmentation.

Periodontists and general practisioners are used on working with high speed, whereas oral surgeons are used to preparing hard tissue utilizing a straight handpiece. One can object that a straight handpiece is causing more vibrations and may dyslodge the shield. However, in our experience (myself:>300 socket shield procedures, Richard Martin, Michael Pikos) it works very well. This is not recommendation for the periodontists and GP to start to prepare socket shield utilizing a straight handpiece, but the explanatory video for oral and maxillo-facial surgeons used on a straight handpiece.

Dental implant success today is judged not only by osseointegration but also by bone, tissue stability and of course long term esthetic results. Cosmetic predictability can often be difficult to attain, and esthetic implant failures can be multifactorial and patient management issues. Once esthetic implant failures occur, many cannot be fully corrected. Some complications must be addressed by an interdisciplinary dental team. In this summary of case reports, surgical considerations are provided, including cases of facial asymmetry/recession due to facial implant placement or bone loss resulting from technique/treatment failures, as well as papillary deficiencies. Restorative considerations for correcting failures are also discussed.

Partial Extraction Therapy (PET) The Final Frontier
Buccopalatal collapse of the postextraction ridge is a significant challenge in restorative and implant dentistry. A variety of different approaches as well as ridge preservation techniques using tissue and augmentative materials have been proposed in the literature. A slightly different approach, "PET" is to use the tooth itself by purposely maintaining all or a portion of the root in the remaining socket. Root submergence has been reported in the literature for more than 4 decades, and it has been demonstrated that the submerged tooth root retains the periodontal tissues and preserves the bone in pontic sites or below dentures to retain the ridge. The socket-shield technique entails preparing a tooth root section to remain behind on the labial aspect simultaneous to immediate implant placement and has demonstrated histologic and clinical results that are highly promising to esthetic implant treatment.

The Socket Shield technique is a “new” procedure with 10 years of documented successful cases worldwide. Lately we have done some new proposals like the way we extract the lingual slice, the relation of the shield level to the bone, the use of miniflaps, the distance between implant and shield just to mention some. On the other hand, the only minor “problem” with this technique is the “external shield exposure” and here I propose the method of successful treatment. We will see the convenience of applying SS to molars also on the light of the paper of Dr. Mataraso from 2009. The molar SS is a viable alternative rather than ridge preservation techniques.

Excessive Gingival Display Managed by Surgical Lip Reposition, Aesthetic Crown Lengthening and Veneers
Excessive gingival display can be managed by a variety of treatment modalities, depending on the etiology. Mild and moderate gummy smiles can be successfully treated by periodontal plastic surgery, however for excessive gingival display as in cases of VME II or III with 5 to 8 mm or more of gingival exposure this procedure is not enough. Many patients decline the more invasive Le Fort osteotomy and opt for a less invasive surgical lip repositioning procedure. Currently Botox is used as alternative, but this is only a temporary treatment with very short effectiveness. That is why we suggest surgical lip repositioning as an effective procedure to reduce gingival display by positioning the upper lip in a more coronal location. The long-term stability of the results remains to be seen, but it holds promise as an alternative treatment to more invasive procedures.

This 50yr.+ female showed external/internal resorption of tooth #11. Socket Shield technique was used saving the facial and interproximal of the tooth. Remainder of tooth was removed by vertically sectioning the tooth with surgical burs in 45 degree high-speed handpiece. Implant placed was a 4.6 x 15mm Tapered Plus Biohorizon implant. Mineross was mixed with saved bone and placed in gap areas. An Emax bonded Maryland bridge was then bonded to adjacent teeth for a temporary replacement. The implant, healing collar and graft were covered with PRF. Nine day post op is shown. This case will be closely followed and posted on XP.

Sequencing of Periodontal Procedures and Orthodontic Treatment
Severe cases of periodontal disease often require periodontal surgery and realignment of teeth. Surgical techniques have been developed that attempt to minimize postsurgical gingival recession and compromise the interdental papillae. A case report is presented in which reversal and correction of a deteriorating maxillary frontal dentition were effectively achieved through combined use of periodontal and orthodontic principles. The treatment plan included the control of periodontal inflammation, restoration of lost attachment apparatus, realignment of anterior dentition, stabilization of occlusion, and minor periodontal plastic surgery. The anticipated loss of a maxillary lateral incisor was avoided. Restoration of a pleasant smile with nicely aligned teeth and esthetic gingival contours was achieved. The correct sequencing of the procedures involved was considered a key factor for the long-term esthetic outcome.
