Hi David, First of all great video, superb explanation!! From what it seems the patient has considerable bony recession associated with gingival recession. So i was wondering if by bringing down some soft tissue to the level of cej you are not putting the patient at risk for gingival pockets? So from an esthetic point of view i understand the gingival root coverage procedure, but from a periodontal point of view, shouldn't that patient get an apical repositioning flap to reduce chances of perio pockets? Thank you

Great video! Thank you very much. Which Perioderm product is shown here? thick vs thin etc...? and could you talk a little bit about the indications for each.

Hi Pascal! Yes, for a connective tissue graft, the same flap design and suturing technique could be used. However, please don't feel limited to the technique I've demonstrated... I routinely use envelope flaps, tunnel flaps, alternating papilla tunnels, etc... I would say that any flap design that works for acellular dermal tissue would work for autogenous tissue as well. Thanks!

Kevin, I've actually never tried Alloderm or Perioderm in this manner, so I can't speak from experience. However, in observing the way these materials have behaved (at least for me) over the past 12 years, I don't think you could leave the material exposed while repairing a buccal plate defect. My feeling (no literature support on this) is that the flap would pull away from the site and the underlying acellular dermal graft would necrose. My preference/suggestion would be to try to obtain primary closure when using this material over buccal defects.

Partial Extraction Therapy (PET) The Final Frontier
Buccopalatal collapse of the postextraction ridge is a significant challenge in restorative and implant dentistry. A variety of different approaches as well as ridge preservation techniques using tissue and augmentative materials have been proposed in the literature. A slightly different approach, "PET" is to use the tooth itself by purposely maintaining all or a portion of the root in the remaining socket. Root submergence has been reported in the literature for more than 4 decades, and it has been demonstrated that the submerged tooth root retains the periodontal tissues and preserves the bone in pontic sites or below dentures to retain the ridge. The socket-shield technique entails preparing a tooth root section to remain behind on the labial aspect simultaneous to immediate implant placement and has demonstrated histologic and clinical results that are highly promising to esthetic implant treatment.

Vertical or Shoulderless Preparations in Contemporary Prosthodontics
Tooth preparations without a defined finish line have been termed in several different ways, such as knife edge, feather edge, or shoulderless. From a biological standpoint, preserving a maximum amount of sound tooth structure, as it is done in vertical preparations, might also offer a more conservative alternative to a horizontal margin design in other clinical conditions such as endodontically treated teeth, vital teeth in young individual, teeth affected by caries at the cervical third. Vertical margins on zirconia crowns have been tested in vitro and clinical reports have been published. A re-evaluation of possible advantages and shortcomings of vertical preparation design in contemporary prosthodontics will be presented in light of recently published literature.

Changes in Soft Tissue Dimensions Following Three Different Techniques of Stage-Two Surgery: A Case Series Report
The aim of this case report is to compare the resilts of the increase in keratinized mucosa using three different techniques of stage-two surgery. Thirty-two patients with one to eight dental implants who received prosthetic rehabilitation of the maxilla were included. Patients were divided into three groups based on preoperative anatomical considerations. Stage-two surgery was performed using wither the apically repositioned flap, the roll flap, or an apically repositioned flap combined with a connective tissue graft.
