Hi Dr Rabie ...nice presentation ..Thank you ...at 8.43 you mention you get screw loosening with screw retained versus cement retained restoration.i don't get this point as a cement retained restoration is attached to the screw retained abutment ..thus biomechanically the same thing.I agree i do more cement retained but not necessarily because they are less likely to loosen? Also you have not suggested the use of 2 standard implants in a molar restoration which would be of even greater surface area and biomechanically superior...do you agree?

Surgical Concepts to Treat Esthetic Implant Disaster Cases - Part 1 of 2
The utilization of dental implants in the esthetic zone is a challenge for involved clinicians. With the increasing application of dental implants by non-experienced colleagues, the occurrence of esthetic failures with implant supported restorations is raising. This lecture will address the causes for such esthetic failures and the surgical approach for therapy. In most cases, implant removal is necessary, which must be done with a low-trauma technique to avoid additional bone loss. Today, this is done with special implant removal instruments using the Reverse Torque Technique. In addition, these failures are often characterized by a lack of keratinized mucosa. The current strategies for the reestablishment of keratinized mucosa and for the augmentation of missing bone, mainly on the facial aspects are presented and documented with case reports. This includes aspects of timing, surgical techniques, and the selection of appropriate biomaterials.

Partial Extraction Therapy (PET) The Final Frontier
Buccopalatal collapse of the postextraction ridge is a significant challenge in restorative and implant dentistry. A variety of different approaches as well as ridge preservation techniques using tissue and augmentative materials have been proposed in the literature. A slightly different approach, "PET" is to use the tooth itself by purposely maintaining all or a portion of the root in the remaining socket. Root submergence has been reported in the literature for more than 4 decades, and it has been demonstrated that the submerged tooth root retains the periodontal tissues and preserves the bone in pontic sites or below dentures to retain the ridge. The socket-shield technique entails preparing a tooth root section to remain behind on the labial aspect simultaneous to immediate implant placement and has demonstrated histologic and clinical results that are highly promising to esthetic implant treatment.

There can always be difficulties in determining the osseointegration status when an implant is to be loaded with the prosthesis. Nevertheless, it is vital to know whether the implant is stable. Titanium endosseous implants have been increasingly used in various edentulous situations for well over a decade. Cigarette smoking has long been suspected as adversely affecting wound healing. Arteriolar vasoconstriction and decreased blood flow are seen in response to smoking. In the oral cavity,an increase in plaque accumulation,a higher incidence of gin-givitis and periodontitis,a higher rate of tooth loss,and an increased resorption of the alveolar ridge have been found among smokers.
