In part 2 of this series, Dr. Ronald Goldstein continues his presentation on an up-to-date look at the role of bleaching in restorative dentistry. In this section Dr. Goldstein covers the current techniques of managing sensitivity during bleaching therapy. In addition, Dr. Goldstein also outlines the clinical methods utilized to evaluate and communicate the relative prognosis for success in treating patients with varied types of shade challenges. Bleaching individual tooth stains is also outlined in detail.

I heard somewhere that dentist could etch the teeth for less than 5 sec between the tow rounds of bleaching..
as i heard it helps absorbing the bleaching material more into enamel prisms...
what do you think of this Dr Ronald ?

The questions about medication afterwards involve 3 types of compounds: MI Paste (GC) which contains Amorphous Calcium Phosphate; and would be my first line of defense and the paatient applies a small amount twice daily; The second compound would be a toothpaste with nitrites in it like Sensodyne(also Flouride containing). and The third alternative would be a compound like Gluma (Heraeus)which I would apply at chairside.

Not sure which patient you refer to but a patient with yellow stains with favorable pulp position would be first treated with an in-office bleaching and then given upper and lower bleaching trays to use at home. You can see the supplies we use if you go into my page under Xperts...all my materials are listed there and we do use different bleaching agents for different patients. Let me know if you need further information.

At-Home Bleaching: A 25-Year Retrospective
The concept of at-home bleaching self-administered by the patient and supervised by the dentist became widely known in the profession about 25 years. At the time, little was known about bleaching except that application of a peroxide-based gel to the teeth could substantially lighten them in 4-6 weeks. Since then, a large volume of laboratory and clinical research have greatly expanded and enhanced our understanding of the process. This presentation provides a brief summary of what we have learned over the last 25 years, including how bleaching works and how long it lasts, safety issues, side effects such as tooth sensitivity, effects on teeth and materials, extended uses for bleaching, and over-the-counter options.

Restorative indications for anterior endodontically-treated teeth have changed, prompting a new interest in bleaching, with new techniques needed for maximum esthetics.
