This requires bone development of the edentulous ridge or potential implant restorative site. Thank you for a great presentation. I am reminded, that with so many approaches, materials, and tools available in regenerative surgery, how important are detailed diagnostics and a thorough understanding of the biology of healing with each one. This will hopefully lead us in choosing the right approach and materials for the specific deformity at hand....A never ending journey in learning.

Alper and Keith; Thanks for the response. You must use ACS Collagen Sponges as carrier for BMP-2 but can mix some mineross with it. As for Sonic Weld system, I have used and continue to use the system but would not have utilized as a substitute for ortho extrusion in the vertical defect case. Dr. Salama

The main factor for soft and hard tissue healing is the speed and quality of new vascularization. This lecture will present all the positive and negative factors who control the angiogenesis, blood supply and bone metabolism: -Biological factors as vit. D and cholesterol have to be checked before the surgery -The management of the flaps with sutures is one the main factor for the bone and soft tissue maintenance.. -The soft brushing technique is a new technique which increases the flap without periosteal incision nor bleeding. -Careful Implant placement without too much torque, specially in cortical bone or grafted bone at the re-entry.. -The A-PRF liquid and i-PRF are solutions to reduce the mobility of the granules (sticky bone) with an improvement of angiogenesis The use of growth factors is a stimulation of angiogenesis. The prevention of the negative factors will allow us to achieve the long term stability.

The use of recombinant human plateletderived growth factor–BB (rhPDGF) has received Food and Drug Administration approval for the treatment of periodontal and orthopedic bone defects and dermal wound healing. Many studies have investigated its regenerative potential in a variety of other oral clinical indications. The aim of this systematic review was to assess the efficacy, safety, and clinical benefit of recombinant human plateletderived growth factor (rhPDGF) use for alveolar bone and/or soft tissue regeneration. Based on the clinical evidence, rhPDGF is safe and provides clinical benefits when used in combination with bone allografts, xenograft, or β-TCP for the treatment of intrabony and furcation periodontal defects and gingival recession or when used with allografts or xenograft for GBR and ARP.

Modified IVAN Technique: Long-Term Follow-Up of 20 Cases Over 2 to 11 Years
When natural teeth fail, frequently there is a loss of hard and soft tissue. This may complicate subsequent dental implant placement by creating insufficient bone to house the implant. This also occurs when the tooth has been missing for an extended period, especially in the premaxilla, where the bone is less dense and often lacks sufficient volume of facial bone. Site reconstruction to accommodate implant placement often requires both hard and soft tissue augmentation. The modified interpositional vascularized augmentation neogenesis (mIVAN) technique achieves the desired treatment goals in both delayed and immediate placement scenarios. The technique will be discussed as well as the long-term follow-up on 20 cases.
