The need for an adequate alveolar ridge width has been a long-standing principle in pre-prosthetic surgery. Since the introduction of root-form endosseous dental implants, the necessity of proper alveolar ridge width has become essential. Many surgical pre-implant bone augmentation techniques with the goal of reconstructing deficient alveolar width, height, or both have been proposed. A variety of alveolar ridge augmentation techniques for treatment of 2-D and 3-D bone deficiencies have been described and include guided bone regeneration (GBR) with a cancellous graft, onlay/veneer block graft, inlay grafting, ridge-split, vertical and horizontal distraction osteogenesis, and others.

There can always be difficulties in determining the osseointegration status when an implant is to be loaded with the prosthesis. Nevertheless, it is vital to know whether the implant is stable. Research today demonstrates that implants can be subject to immediate or early loading, but there were no objective standard methods of determining implant stability which will be very helpful criteria for immediate or early loading. Our objective in these 2 sequential lectures was to provide straightforward decision trees related to several clinical scenarios observed in routine clinical practice. The 3 fundamental pillars for peri-implant excellence are: restorative-driven implant position, hard and soft tissue reconstruction using different biomaterials and grafts, and prosthetic management. Using this philosophical approach seems to provide esthetic and stable results over time irrespective of the initial clinical condition.

Proper diagnosis, extraction technique, implant selection, and placement significantly impact outcomes. Respect of hard and soft tissue physiology following tooth loss and implant placement requires specific steps be taken. Management tissue contours properly, via regenerative therapy, results in successful framing of the restoration. Provisionalization and definitive restorative therapy also impacts the level of esthetic success.The contours of the temporary abutment and crown develop soft tissue contours for the final restoration. Overcontouring can lead to soft tissue recession and mucosal asymmetry. Design of the definitive crown(s) is crucial for long-term maintenance of esthetically acceptable results.
