Dr. Alvarez, sin tener nada que ver con su profesión, como una ama de casa común y silvestre, quisiera agradecer la forma tan clara y educativa de expresar su técnica. En 2o lugar quisiera preguntarle sobre el tipo de cirugía. Tengo 51 años y he perdido varias piezas dentales, no por falta de higiene, más bien lo contrario, mis dientes son pequeños y al hacerme limpiezas dentales el especialista hizo zurcos que posteriormente causaron que mis dientes se "partieran", sin contar los que ya habían sido tratados anteriormente con las técnicas apropiadas según los especialistas que me trataron. La pregunta va por que generalmente soy muy alérgica a diferentes materiales. Puede mi encía y organismo causar un rechazo a los fijadores de la prótesis? No soy hipertensa hasta ahora, pero la cirugía presenta risgos en ese sentido? A mi me gustaría volver a comer por ej, carne como antes, preo me siento sumamente egoista pensando en mis dientes cuando pienso por ej. en mi hermana menor que vive en Canarias, ella nació con labio leporino y hendidura palatina y aún no puede tener esa dentadura definitiva por falta de recursos, por ello le pregunto, es muy caro el procedimiento en ambos casos? Disculpe Ud. la franqueza, pero por su clase observé que Ud. tiene por prioridad el bienestar del paciente, casi siempre el problema son los costos, tanto en mi caso como en el de mi hermana allá. Jose, disculpa la tardanza en la respuesta, el modelo se realiza a partir de la guía quirúrgica entonces hay que estar muy encima de todo el proceso de laboratorio a fin de evitar errores que después se pagan muy caros, sobre todo en la construcción de modelos

Excelente presentación! Dr.el modelo de trabajo con analogos se confeccionó a partir del la guía quirúrgica? Habría que hacer una "cirugía" al modelo? La compañia lo envia o toma la impresión para carga inmediata postquirúrgica? Gracias

Fatigue Resistance and Crack Propensity of Novel "Super-Closed" Sandwich Composite Resin Restorations in Large MOD Defects
To asses the influence of conventional glass ionomer cement (GIC) vs resin-modified GIC (RMGIC) as a base material for novel, super-closed sandwich restorations (SCSR) and its effect on shrinkage-induced crack propensity and in vitro accelerated fatigue resistance. A standard MOD slot-type tooth preparation was applied to 30 extracted maxillary molars. A modified sandwich restoration was used, in which the enamel/dentin bonding agent was applied first, followed by a Ketac Molar or Fuji II LC base, leaving 2mm for composite resin material. Shrinkage induced enamel cracks were tracked with photography and transillumination. Samples were loaded until fracture or to a maximum of 185,000 cycles under isometric chewing, starting with a load of 200 N and gradually up to a maximum of 30,000 each...
