Immediate Molar Implants: When, Why, and How?

Description:
Multi-rooted extraction sockets are often too large to accommodate a traditional diameter implant, therefore ultra wide diameter implants as a clinical alternative can predictably simplify procedures without compromising function and esthetics. Problems with use of traditional implants may include the ability to gain adequate primary stability, use of large amounts of graft material, and poor embrasure spaces causing food impaction. The focus of this lecture will highlight clinical rational to improve success, anatomical and restorative benefits of immediate placement, evidence-based outcomes reported in literature, and survival and success rates reported by the presenter. This lecture is intended for restorative dentists.

Very interesting presentation and the decay remark needs in my opinion to check more parameters than just the implant - tooth distance. A lot of time I notice a loss of the contact point after some years who was for sure present at the crown placement . Consequence is proximal food retention and decay . But I will try to observe in my cases your thoughts on that . Bravo !

Crestal Sinus Approach: A Hammerless Solution to the Indirect Sinus Lift
Posterior maxillary dental implant reconstruction for advanced alveorlar ridge atrophy has become possible through bone grafting procedures involving the maxillary sinus. This procedure involves augmentation of either the internal or external aspects of the sinus, or both. Bone grafting of the external aspect is performed with guided bone regeneration, using allogenic, autogenous, cortical, or corticocancellous grafts, as well as growth factors such as PRF as onlays with immediate or delayed implant placement. With the advent of new surgical techniques and equipment, sinus augmentation has become a predictable and efficient treatment. This presentation will focus on the internal, hammerless approach.

Post-Traumatic Treatment of Maxillary Incisors by Immediate Dentoalveolar Restoration with Long-Term Follow-Up
Replacing both missing maxillary interior teeth is particularly challenging, especially in compromised sockets. The case report describes the management of an 18-year-old female patient, who suffered avulsion of both maxillary central incisors at 7 years of age. The multidisciplinary implant technique, called Immediate Dentoalveolar Restoration (IDR), included extraction of the injured teeth and a single procedure for immediate implant placement and restoration of the compromised sockets after root fracture and peri-apical lesion development were detected during orthodontic treatment. Successful esthetic and functional outcomes and reestablishment of the alveolar process after bone reconstruction were observed during the 3-year follow-up period. The predictable esthetic outcomes and soft and hard tissue stability that can be achieved following IDR are demonstrated.

Survival of Implants after Indirect Maxillary Sinus Elevation Procedure: A Two Years Longitudinal Study
The aim of the study was to evaluate the survival rate of two diverse implant systems with different implant surfaces with the same geometrical design. In the present study, we achieved clinical success with both kinds of implant surfaces however Bioetched implant surface showed promising results comparable to Tiunite surface of Nobel BioCare Implants. In the future, more case-controlled studies with longer follow-up are needed to validate the results of the present findings.

Ridge Preservation with Modified “Socket-Shield” Technique: A Methodological Case Series
After tooth extraction, the alveolar bone undergoes a remodeling process, which leads to horizontal and vertical bone loss. These resorption processes complicate dental rehabilitation, particularly in connection with implants. Various methods of guided bone regeneration (GBR) have been described to retain the original dimension of the bone after extraction. Most procedures use filler materials and membranes to support the buccal plate and soft tissue, to stabilize the coagulum and to prevent epithelial ingrowth.
