Ed; My goal was 2mm labial to the implant. I now use a mixture of 50:50% Cortical-Cancellous Mineralized Bone or I collect autogenous bone with the ACM (Auto Chip Maker) drills. Dr. Salama

dr. salama. what was your goal in mm in regeneration of the buccal bone. i use only mineross for my GBR procedures; should i be using straight cortical bone on the outer aspect; a layering technique? sounds like more bone regeneration that way. thanks.

Kevin; The best way is to purchase a Neo-GBR kit from Neobiotech and then purchase the CTi-mesh separately. Secure screws on both buccal and lingual and a tenting screw below the mesh to maintain the space. You can purchase these items directly on the Products page and click on Neobiotech. Good luck Dr. S

I want to use the cti mesh from neobiotech to get some vertical height regenerated. but i want to stage it. what size screws would you recomend to secure it to the buccal and lingual? or would i just need to secure it to one side?

Mark; I have not utilized Cytoplast Ti reinforced for this procedure. I would be reluctant to as it does not allow for the same space maintaining as does Titanium mesh only. I too think Cytoplast membranes are good options for buccal dehiscence of an extraction site but I also like the Socket repair membrane of Zimmer for this indication. regards Dr. Salama

Dr. Salama - Have you ever or do you know of any techniques similar to this but using titanium reinforced cytoplast? I have thought about this in the past. Using the cover screw or low profile healing abutment to secure the cytoplast. That way if there is some breakdown over the cover screw, which is more of a problem in the lower, the exposure may be tolerated better. I have had good success using that membrane for buccal dehiscence after ant extraction and not getting primary closure, but have not had great success around implants. Would securing with the screw help with stability and better results? I am thinking about using primarily when you have a small buccal exposure of the implant on the lower but good primary stability otherwise. thanks.

Dr. Eldardir; Thank you. It is best to extend the envelope flap full thickness in the area until you can see the exit of the nerve from the foramen reaching out into the flap. An depression in the labial wall typically extends beyond the nerve. Due this with blunt dissection only. This is dispalyed beautifully on Dr. Pikos video on XP for mandibular block graft. Maurice,thank you for your answer and this another fantastic case presentation.No,I haven't tried the PRGF techniques as yet.I see it is a routine practice for most of the experts on Dental XP and the results are excellent.

Socket Gafting with the use of Autologous Bone - an Experimental Study in the Dog
In five beagle dogs, the distal roots of the third and fourth mandibular premolars were removed. The sockets in the right or the left jaw quadrant were grafted with either anorganic bovine bone or with chips of autologous bone harvested from the buccal bone plate. After 3 months of healing, biopsies of the experimental sites were sampled, prepared for buccal–lingual ground sections and examined with respect to size and composition.

Guided Bone Regeneration Using a Titanium Membrane at Implant Placement: A Case Report and Literature Discussion
Reconstruction of the oral supporting tissues lost by disease or trauma is essential to tooth replacement with dental implant therapy. This treatment requires evidence based augmentative procedures combined with up-to-date and current techniques. Guided bone regeneration (GBR) aims to initialize this process of alveolar ridge reconstruction by utilizing biologically active and supportive materials best coupled to the body’s healing processes. The use of nonresorbable, titanium membranes can achieve GBR by ensuring graft stability and space maintenance so as to ensure optimal neovascularization. Hereafter is a case report of a ridge defect reconstructed at implant placement, with the rationale and current, evidence-based literature discussed.
