Description:
In this instructional lecture recorded at a recent symposium in Florida in June 2009, Dr. Nicolas Elian, Assistant Professor and Head of the Division of Implant Dentistry at New York University College of Dentistry, outlines protocols for site assessment and enhancement for successful esthetic implant therapy. The single tooth site, as well as larger edentulous sites are discussed. In addition, a very detailed description is given on the utilization of bone grafting with or without membranes, including the use of titanium mesh, to achieve predictable 3 dimensional enhancement.

A GRAFT-LESS Approach to Implant Dentistry
Vertical bone augmentation techniques are more invasive and success rates often vary according to the surgical expertise of the clinician. There has been a trend over time towards minimally invasive treatment options in implant dentistry. This approach attempts to avoid more complicated procedures involving bone augmentation for implant placement. This presentation discusses a “graft less” treatment philosophy that emphasizes the use of less demanding augmentation techniques for the purpose of placing short implants in atrophic posterior sites.

Teeth First: A novel surgical treatment concept for the edentulous, acquired Class III, severely atrophied maxilla
Extreme atrophy of the edentulous maxilla still represents a challenge as far as prosthetic rehabilitation with implant-supported fixed (screw-type) prostheses is concerned. This is due not only to insufficient bone volume for implant placement but also to unfavorable three-dimensional inter-maxillary relationships that is caused by the pattern of bone resorption in the maxilla. In the scientific literature, there are few clinical studies describing the use of the Le Fort I osteotomy, as a 1 or 2 stage procedure, for the reconstruction of a three dimensionally severely atrophied maxilla, prior to implant placement.
